摘 要:目的:测定正常颊黏膜组织、糜烂型及非糜烂型口腔扁平苔藓(Orallichen planus,OLP)组织和颊癌组织的微血管密度(Microvessel density,MVD),观察其MVD的变化,为进一步探讨OLP的发病机理及治疗方法提供参考。方法:选择2008年12月至2010年3月在重庆医科大学附属口腔医院就诊的初诊患者共40例,包括10例糜烂型OLP,10例非糜烂型OLP和10例颊癌,收集10例颊部外伤创面组织作为正常人颊黏膜标本。组织切片采用CD34标记血管内皮细胞,S-P免疫组化染色,光学显微镜下测定MVD,用SPSS软件对数据进行统计学分析。结果:颊癌、糜烂型OLP、非糜烂型OLP、正常颊黏膜的MVD分别为:36.825 0±7.048 5、27.700 0±6.183 3、16.600 0±5.111 57、.350 0±1.472 9,经秩和检验分析,糜烂型OLP与正常对照组之间、颊癌与正常组织之间、颊癌与非糜烂型OLP之间的MVD差异有统计学意义(P<0.05)。结论:糜烂型OLP及颊癌较正常颊黏膜组织MVD明显增高;糜烂型与非糜烂型OLP的MVD差异无统计学意义;MVD与OLP组织的病变程度有明显相关性,可以考虑将MVD作为OLP治疗转归、癌变发展的评价指标。Objective:To detect the microvessel density ( MVD ) in tissue of normal buccal mucosa, erosive and non-erosive oral lichen planus and buccal cancer; to observe the microvessel density changes in order to further explore the pathogenesis of oral lichen planus and provide reference treatment. Methods: A total of 40 cases of newly diagnosed patients were selected from December 2008 to March 2010 in the affiliated hospital of stomatology, Chongqing medical university, including 10 cases of erosive OLP, 10 cases of non-erosive OLP and 10 cases of buccal cancer, and 10 cases of buccal injury wound tissue collected as a normal buccal mucosa specimens. Tissue sections were performed by using vascular endothelial cell markers CD34, S-P immunohistochemistry.The microvessel density was measured under the optical microscope, with SPSS software for statistical analysis. Results:The MVD data for buccal cancer, erosive OLP, non-erosive OLP, and normal buccal mucosa were 36.825 0 ± 7.048 5,27.700 0 ± 6.183 3,16.600 0 ± 5.111 5, and 7.350 0 ± 1.472 9 respectively;by rank sum test, the difference of MVD between the erosive OLP and normal buccal mucosa, buccal cancer and normal buccal mucosa, buccal cancer and non-erosive OLP groups had statistical significance ( P 〈0.05 ). Conclusion : The MVD of erosive OLP and buccal cancer is much higher than that of normal oral buccal mucosa;there's no significant difference in MVD between the erosive and non-erosive OLP; the microvessel density and the severity of OLP tissue are significantly correlated, which could be considered as an evaluating index for the treatment and prognosis of OLP, as well as the development of cancer.
