上颌骨功能性重建中用钛植入体重建颧上颌支柱的生物力学研究

摘　　要：目的:对腓骨+不同钛植入体的上颌骨重建的三维有限元模型进行生物力学分析,研究上颌骨功能性重建中用钛植入体重建颧上颌支柱的可行性。方法:分别对腓骨+重建钛板、腓骨+钛网和腓骨+颧种植体3种上颌骨重建的三维有限元模型,于双侧后牙区垂直加载,研究钛植入体的应力和位移分布。结果:腓骨+重建钛板和腓骨+钛网这2种类型在钛板和钛网的转折处应力和位移最大;腓骨+颧种植体在颧种植体植入颧骨处的应力最大,在颧种植体的中点位移最大。结论:在上颌骨功能性重建中,用颧种植体重建颧上颌支柱是可行的,而用钛板或钛网重建颧上颌支柱则不可行。PURPOSE： To study the feasibility of zygomatic-maxillary buttress restoration with titanium alloplastic implant in functional maxillary reconstruction by performing biomechanical analysis of three-dimensional finite element models of neomaxilla reconstructed with fibula and different titanium alloplastic implants.METHODS： Stress and deviation distributions of titanium alloplastic implant were studied by application of vertical load on bilateral molar regions in different three-dimensional finite element models of neomaxilla reconstructed with fibula and reconstruction plate,fibula and titanium mesh,and fibula and zygomatic implant.RESULTS： The highest stress and the largest deviation were both at the turning point of reconstruction plate and titanium mesh in two types of models reconstructed with fibula and reconstruction plate,and fibula and titanium mesh.The highest stress was at the region of zygomatic implant on the zygoma and the largest deviation was at the midpoint of zygomatic implant in the model reconstructed with fibula and zygomatic implant.CONCLUSIONS： In functional maxillary reconstruction,restoration of zygomatic-maxillary buttress with zygomatic implant is feasible,and restoration of zygomatic-maxillary buttress with reconstruction plate or titanium mesh is infeasible.
