腹腔镜结直肠手术的暴露技巧

摘　　要：目的：探讨腹腔镜结直肠手术的暴露技巧。方法：回顾分析2003年7月至2010年12月施行429例腹腔镜结直肠手术的临床资料。结果：本组381例成功完成腹腔镜手术,成功率88.8%,中转开腹48例,中转率11.2%。手术时间95～265m in,平均146m in,术中均未输血。所有患者均对手术效果满意,术后3～8d痊愈出院。术后随访4～53个月,平均37个月。结论：腹腔镜结直肠手术通过合适的术前准备、体位暴露、器械暴露、系带悬吊和助手协助暴露等方法可实现对手术部位的暴露。明显降低腹腔镜结直肠手术的操作难度,具有患者创伤小、康复快等优点,越来越受到重视。Objective：To investigate the exposure of the laparoscopic colorectal surgery.Methods：The retrospective analysis was made on the clinical data of 429 patients who underwent the laparoscopic colorectal surgery from Jul.2003 to Dec.2010.Results：The operation time was 95-265min（mean,146min）.No case was given blood transfusion during operation.48 cases（11.2%） were transferred to opening operation.All patients were satisfied with the operation outcome.The period in hospital was 3-8d.The patients were followed up for 4 to 53 months（mean,37 months）.Conclusions：The exposure of visual fields of the laparoscopic colorectal surgery can be completed by sufficient preparation of preoperation,posture,appliance,frenum and assisting of adjutant.The sufficient exposure can obviously reduce the difficulty of the laparoscopic colorectal surgery.
