腹腔镜结直肠癌根治术并脐静脉置泵化疗预防术后肝转移的临床研究

摘　　要：目的:研究腹腔镜结直肠癌根治术后经脐静脉化疗泵持续灌注5-FU对减少术后肝转移、提高远期疗效的作用。方法:为106例患者行腹腔镜结直肠癌根治术,随机分为脐静脉化疗组(研究组,n=53)和对照组(n=53)。研究组术中经脐静脉向门静脉插管,术后即从脐静脉化疗泵持续给予5-FU 1g/d,连续7d。对比分析两组近期并发症和远期疗效。结果:两组术后住院时间及术后近期并发症差异无统计学意义(P>0.05),研究组术后3年肝转移率明显低于对照组(12.3%vs.32.9%,P=0.023),而3年生存率显著高于对照组(83.6%vs.63.9%,P=0.033)。结论:腹腔镜结直肠癌根治术后即经脐静脉化疗泵持续灌注5-FU能明显降低肝转移,提高生存率。Objective：To investigate the effect of 5-Fluorouracil（5-FU） infusion through umbilical vein pump for 7 days on preventing liver metastasis and improving the survival rate after laparoscopic radical resection for colorectal cancer.Methods：The 106 colorectal cancer patients with laparoscopic radical resection were divided into portal vein chemotherapy group（study group,n=53） and control group（n=53） randomly.In study group,5-FU were infused 1.0g per day constantly for 7 days after operation through umbilical vein pump,the 5-FU was placed into portal vein through umbilical vein.The short-term complications and long-tem effect were compared between the two groups.Results：There were no significant differences in hospital stay and short-term complications between the two groups（P0.05）.The 3-year liver metastasis rate in study group was significantly lower than that in control group（12.3% vs.32.9%,P=0.023）,and the 3-year survival rate in study group was significantly higher than that in control group（83.6% vs.63.9%,P=0.033）.Conclusions：5-FU consistent infusion through umbilical vein pump after laparoscopic radical resection for colorectal cancer can lower liver metastasis and improve survival rate.
