HBV-M阳性前S1蛋白临床上检测分析

摘　　要：目的:检测HBsAg前S1蛋白,评价其在病毒感染、复制、传染及对肝细胞损伤等方面的临床意义。方法:根据血清标志物(HBV-M),将298例HBsAg感染患者分为4组,即HBsAg(+)组(A组),HBsAg(+)-HBeAg(+)-HBcAg(+)组(B组),HBsAg(+)-HBcAb(+)-HBeAb(+)组(C组),HBeAg(+)-HBcAg(+)组(D组)。分别对各组进行前S1蛋白、HBV-DNA及丙氨酸转氨酶(ALT)检测,并对结果进行分析。结果:前S1蛋白阳性共140例(46.98%),其中A组12例、B组50例、C组60例、D组18例;HBV-DNA阳性共119例(39.93%),其中A组4例、B组42例、C组51例、D组22例;ALT异常病例共124例(41.61%),其中A组3例、B组45例、C组55例、D组21例。前S1蛋白阳性和HBV-DNA阳性发生率各组间差异不显著(P>0.05),两者相关系数0.98,χ2检验结果无显著性差异(P<0.01)。结论:前S1蛋白检测阳性,不论HBeAg与ALT检测结果是否正常,都表示存在乙肝病毒复制并具有传染性。乙肝病毒前S1蛋白可作为判断乙肝病毒感染、复制、传染性及肝细胞损伤的指标。Objective： To evaluate the clinical value of Pro-S1 protein in detection of HBV replication,infectivity and liver cell injury.Methods： A total of 298 cases with positive HBsAg were divided into 4 groups according to serum index（HBV-M）： HBsAg（＋） Group（Group A）,HBsAg（＋）-HBeAg（＋）-HBcAg（＋） Group（Group B）,HBsAg（＋）-HBcAb（＋）-HBeAg（＋）（Group C）,and HBeAg（＋）-HBcAg（＋）（Group D）.Pre-S1 protein,HBV-DNA and ALT of each group were determined and analyzed.Results：Among 298 cases,140 cases were positive in Pre-S1 protein test（46.98%）,including 12 cases in Group A,50 in Group B,60 in Group C and 18 in Group D;119 cases were positive in HBV-DNA test（39.93%）,including 4 cases in Group A,42 in Group B,51 in Group C and 22 in Group D;124 cases showed abnormal results of ALT level（41.61%）,including 3 cases in Group A,45 in Group B,55 in Group C and 21 in Group D.There were no significant difference in the positive rates of pre-S1 protein and HBV-DNA（P0.05）.The correlation coefficient was 0.98.The χ2 test showed no significant difference（P0.01）.Conclusions： If the result of Pre-S1 protein detection is positive,no matter the results of HBeAg and ALT are normal or abnormal,HBV has the capability of replication and infection.Pre-S1 protein could be regarded as a criterion of infection,replication,infectivity and liver cell injury of HBV.
