白内障超声乳化及人工晶体植入术的临床研究

摘　　要：目的：评价白内障超声乳化及人工晶体植入的方法、手术技巧及术中、术后并发症的防治。方法：对年龄在１１～８８ 岁共１７２ 例（２００ 眼）白内障行超声乳化摘除及人工晶体植入术。结果：平均乳化时间４ 分４８ 秒，平均超声能量３５％ 。术后１ 周视力≥０．５ 者１５６ 眼（７８％ ），术后１ 个月视力≥０．５ 者１９３ 眼（９６．５％ ），其中≥１．０ 者８８ 眼（４４％ ）。术后１ 个月平均角膜散光与术前相比差异无显著性（Ｐ＞ ０．０５）。术中主要并发症是后囊膜破裂和虹膜损伤及术后角膜内皮混浊。结论：该种手术视力恢复快。Objective:To evaluate the surgical methods and technique for phacoemulsification and the intraocular lens (IOL) implantation,prophylaxis and treatment of complications.Methods:The surgery was performed on 200 eyes of 172 patients with cataract,whose age was 11～88.Results:The average phaco time was 4min and 48s, and ultrasonic energy was 35%.On the 7th day postoperatively,156 eyes(78%)reached the vision of≥0.5 and 193 eyes(96.5%)achieved the same vision on the 30th postoperatively,among them the vision of 1.0 or better was found in 88 eyes(44%).There was no statistical difference in mean astigmatism between preopertion and postoperation on the 30th after the operation.The rupture of posterior capsule and iris injury were main operative complications and corneal endothelial opacity was the main postoperative complication.Conclusion:The restoration to vision after the surgery is much better than that after routine cataract precedure.The key to success in phacoe mulsification and IOL implantation is the practised surgical methods and technique.
