摘  要:目的探讨儿童及青少年胶质瘤组织中核苷酸交叉互补基因1(ERCC1)、B细胞特异单克隆鼠白血病毒整合位点基因1(BM I-1)的表达与胶质瘤放疗敏感性及预后的关系。方法收集2000年1月-2006年2月初诊接受手术、单纯放疗或同期放化疗、由中山大学肿瘤防治中心病理确诊为胶质瘤的儿童及青少年患者肿瘤石蜡标本85例。采用免疫组织化学二步法检测ERCC1、BM I-1蛋白的表达,统计分析二者与胶质瘤患者临床病理特征、放疗疗效、术后总生存率的关系。结果 ERCC1、BM I-1蛋白在85例胶质瘤组织中的阳性率分别为32.9%(28例)、37.6%(32例);BM I-1在高级别胶质瘤中的表达显著高于低级别,差异有统计学意义(P=0.027)。BM I-1、ERCC1高表达患者放疗有效率明显低于低表达患者(P=0.020,0.024)。Kaplan-Meier生存分析显示ERCC1、BM I-1蛋白高表达患者术后总生存时间短于低表达患者(P=0.028,0.006)。多因素Cox回归分析显示病理级别、术后行为状态评分、放疗疗效、BM I-1的表达是青少年儿童脑胶质瘤患者独立的预后因素(Pa<0.05)。结论 ERCC1、BM I-1蛋白表达与胶质瘤放疗疗效相关,BM I-1蛋白高表达与胶质瘤的恶性程度及预后相关,是青少年儿童胶质瘤患者的独立预后指标。Objective To analyze the relationship between the expressions of excision repair cross complementation group 1(ERCC1),B-cell-specific monoclonal leukemia virus insert site 1(BMI-1) and the effect of radiotherapy and prognosis of glioma in children and youngsters.Methods Tumor samples were obtained from 85 children and youngsters′ patients with pathologically confirmed glioma at the Cancer Center of Sun Yat-Sen University.From Jan.2000 to Feb.2006 follow-up data were enrolled in this study,and all of the individuals with preliminary diagnosis received concurrent chemoradiotherapy.The presence of ERCC1 and BMI-1 were measured by immunohistochemistry in 85 children and youngsters′ glioma tissues.The data were analyzed according to the clinic pathologic features and follow-up.Results Twenty-eight cases of 85 cases(32.9%) showed high ERCC1 expression,and 32 cases of 85 cases(37.6%) showed high BMI-1 expression.There were dramatic differences in BMI-1 expression between high and low grades of pathology(P=0.027).The number of BMI-1 positive cells were much more in low grade glioma than in high grade one.High expressions of ERCC1 and BMI-1 were closely related with worse effect after radiotherapy in children and youngsters′ glioma(P=0.020,0.024).The Kaplan-Meier analysis showed that there were significant differences in overall survival between the high expression and low expression groups of ERCC1(P=0.028) and BMI-1(P=0.006).Multivariate Cox proportional hazards regression analysis indicated that histological grade,karnofsky performance status score,radiotherapeutic effect,and BMI-1 expression were all independent prognostic factors for overall survival(Pa0.05).Conclusions ERCC1 and BMI-1 are closely related to the resistance of radiotherapy of glioma in children and youngsters.BMI-1 positive cell number is associated with the malignancy and prognosis of glioma,and its expression is an independent prognostic factor for overall survival in children and youngsters.
