摘 要:目的探讨和比较彩色多谱勒超声(color doppler flow imaging,CDFI)及全数字化乳腺摄影(full-field digital mammography,FFDM)诊断乳腺良恶性疾病的准确率。方法依据CDFI及FFDM的影像学特征对254例乳腺肿瘤病例的298个病灶行良恶性诊断学分组,良性病变组86例128个病灶,恶性病变组168例170个病灶,均与病理学诊断结果相对比。结果 CDFI及FFDM将298个病灶分组为可能良性,性质待定和可能恶性;以病理学诊断为确诊依据,128个病灶的良性组CDFI及FFDM诊断准确率分别为92.19%和75.00%,CDFI的误诊率(3.13%)低于FFDM(9.38%),有诊断学优势;170个病灶的恶性组CDFI及FFDM诊断准确率分别为90.00%和87.65%,但FFDM的误诊率(4.71%)低于CDFI(7.65%)。结论 CDFI及FFDM在良恶性病变诊断中有互补作用。CDFI、FFDM及两者联合应用是诊断乳腺良恶性病变实用的影像学技术。Objective To evaluate the accuracy of CDFI and FFDM for diagnosis of breast lesions. Methods 298 breast lesions ( 128 benign and 170 malignant) in 254 patients were prospectively classified according to the imaging features by CDFI and FFDM. The results were compared with pathological results. Results The groups were classified as probably benign, indeterminate and probably malignant respectively by CDFI and FFDM. We considered the pathological results as standard, the accuracy of diagnosis in benign group in 86 patients were 92. 19% and 75.00% respectively by CDFI and FFDM, The accuracy of diagnosis in malignant group in 170 patients were 90.00% and 87.65% respectively by CDFI and FFDM, CDFI and FFDM could complement at diagnosis in benign and malignant breast lesions. Conclusions CDFI and FFDM are valuable technique in benign and malignant breast lesions, combination of the CDFI and FFDM can improve the diagnostic accuracy to breast lesions.
