超声时钟法定位诊断视网膜脱离在离体猪眼中的对比研究

摘　　要：目的通过对离体猪眼中视网膜脱离的超声和解剖后显微镜下的对比观察,验证超声时钟法定位诊断视网膜脱离的准确性和获得视网膜脱离的感性认识。方法 (1)将离体猪眼放入水槽中进行预实验,获得离体猪眼超声观察的可行性。(2)对40只离体猪眼中可清晰辨认视网膜脱离的32只眼用不同颜色标记的四个图钉对称固定后,分别模拟12、3、6、9时钟钟点后进行观察。(3)对离体猪眼中观察到的视网膜脱离运用时钟法进行定位诊断。(4)对离体猪眼逐层解剖后,重点观察脱离视网膜的实物并与超声声像图对比。结果 (1)离体猪眼及离体猪眼中视网膜脱离的声像图与活体人眼声像图极相似。(2)离体猪眼视网膜脱离的时钟法超声与解剖后定位诊断结果符合率为88%(28/32)。(3)离体猪眼中脱离的视网膜呈一侧紧邻视乳头的、边缘不整的、高低不一的、范围不等的、半透明的泡状、膜状或漏斗状隆起,上有细小淡红色血管分布。结论超声时钟法定位诊断视网膜脱离经过离体猪眼中的实验观察以及眼科解剖后显微镜下实物对比证实,可在临床中运用和推广。Objective To test the exactitude of the level diagnosis of retinal detachment with clock location by ultrasound and acquire the practical knowledge of retinal detachment after the ultrasonogram of retinal detachment in isolated pig eyes compared with the expressions of the anatomized isolated pig eyes in microscope.Methods 1.Preliminary test was performed in isolated pig eyes with water wrapped.2.32 isolated pig eyes with retinal detachment（in 40 isolated pig eyes） affixed with four colours thumbtack were observed after simulated clock pointed to 12,3,6 and 9 respectively.3.Retinal detachment in isolated pig eyes were level diagnosis with clock location.4.It was important to observe and level diagnosis the retinal detachment in anatomized isolated pig eyes and contrast with ultrasound.Results 1.The ultrasonogram of retinal detachment in isolated pig eyes was similar to the living human ultrasonogram.2.About 88% of the level diagnosis of retinal detachment in isolated pig eyes by ultrasound were coincident which dianosis by ophthalmologic examination with clock location.3.The practicality of retinal detachment in isolated pig eyes were showed semitransparent follicular,membranous swells or funnel which was close to optic papilla with different edge,range and minute vessel.Conclusion The ultrasonic level diagnosis of retinal detachment with clock location could popularize and utilize after verified by the practicality and the diagnosis of retinal detachment in isolated pig eyes.
