Treatment effect of maxillary protraction on skeletal class Ⅲ high angle cases in mixed dentition

摘  要:目的探讨上颌高位前方牵引联合上腭快速扩弓矫治生长发育期骨性Ⅲ类高角型患者的临床效果。方法对高角型骨性Ⅲ类错,且伴有上颌骨深度发育不足,并根据矫治前左手腕X线片及头颅侧位片颈椎发育程度判断,均处于生长发育期的21例患者先行上腭快速扩弓,之后开始前方牵引,牵引力作用于口外平面上方18 mm处,分析矫治前后头颅侧位片的相关指标。结果上颌骨向前生长,并且未发生旋转,上颌牙合平面顺时针旋转,上前牙舌倾伸长。结论口外高位上颌前方牵引力,能有效避免上颌骨逆时针旋转,其作用线接近上颌骨阻抗中心,可用于高角型或伴有前牙开牙合倾向的骨性Ⅲ类患者。Objective To investigate the clinical effects of combined rapid maxillary expansion(RME) and maxillary protraction appliance in the treatment of skeletal class III high angle cases.Methods 21 cases of high angle skeletal class III malocclusions with maxillary retrognathism were treated using RME and face mask.The results showed that the growth and development stages of cases were evaluated according to x-ray film of left wrist and lateral cephalometric radiograph of cervical vertebra.The force was applied externally 18 mm above the maxillary occlusal plane.Lateral cephalometric radiographs of 21 patients were examined.Results The results showed the force systems were effective to protract the maxilla.We observed forward growth of maxilla without rotation;the maxillary occlusal plane demonstrated a clockwise rotation;the maxillary incisors were retroclined and extruded.Conclusion The force application from near the center of resistance of the maxilla was an effective method to prevent counter-clockwise rotation of the maxilla.The results suggest that this method can be used effectively on patients who present as high angle class III combined with an anterior open bite.
