超选择子宫动脉灌注化疗对宫颈癌细胞超微结构的影响研究

摘　　要：目的通过观察宫颈癌超选择子宫动脉灌注化疗(介入化疗)后临床症状、肿瘤局部及超微结构动态变化,探讨超选择子宫动脉灌注化疗的疗效。方法对4例病理证实为宫颈鳞状细胞癌的病例,给予双侧子宫动脉灌注化疗,于介入前、后动态观察肿瘤局部及肿瘤细胞超微结构变化。结果 4例病例临床疗效均为部分缓解(PR)。超微结构改变为:(1)细胞间隙增宽,桥粒连接减少、结构发育差,微绒毛减少或消失;(2)细胞器改变:内质网肿胀形成囊泡、囊池,线粒体固缩、肿胀,溶酶体增多;细胞核固缩、溶解、破裂、异染色质增加、核浆比例缩小;(3)出现细胞肿胀、凋亡小体、凋亡细胞等凋亡形态表现;(4)周围细胞反应:炎细胞浸润及成纤维细胞较明显。结论超选择子宫动脉灌注化疗使宫颈肿瘤细胞超微结构改变,出现明显癌细胞凋亡现象,最终达到治疗目的 。Objective Through observing clinical symptom,the whole of tumor and ultrastructure kinesis changed after pre-operational interventional therap,to approach the mechanism of action about it.Meth-ods Four patients through patho-confirmed in really cervical squamous cell carcinoma（SCC） was given pre-operational interventional treatment.All patients accord to time of intervention ago and after in the clinical and tumour part and cell ultrastructure way kinesis changes were observed.Results The curative effect of clinical,all of 4 case for partial remission（PR）.The changes of ultrastructure： cell spaces widen architectonic growth badly microvilli（MV） decrease or abolition.The cell organelle changed： endocytoplasmic reticulum engorge for encystation,balloon pool,mitochondrion pycnosis,engorgement,cytolysosome mult,cell nucleus pyknosis,dissolve,disrupt,chromatospherite decrease relative,heterochromatin increase karyoplasmic ratio deflate,cellular swelling,apoptotic body appearance to frequency;Inflammatory cell increased.Conclusion Pre-operational interventional therapy,clinical can improve symptom,deflate gross tumor volume,destroy,tumour cell ultrastructure and change apoptosis,which can achieve treated objective in fine.
