子宫动脉灌注化疗对Ⅱ期宫颈癌组织Survivin蛋白表达的影响及临床意义

摘　　要：目的探讨子宫动脉灌注化疗对Ⅱ期局部晚期宫颈癌组织Survivin蛋白表达的影响及临床意义。方法选择2005年1月~2007年12月在我院住院治疗的Ⅱ期局部晚期宫颈癌患者77例,均为初治病人,采用免疫组化方法检测子宫动脉灌注化疗前后Survivin的表达情况。结果全组病例子宫动脉灌注化疗总有效率64.93%,其中Ⅱa期有效率为61.11%,Ⅱb期有效率为66.10%;子宫动脉灌注化疗前后Survivin阳性表达率63.64%和44.15%,差异有统计学意义（P〈0.05）;化疗前后的Survivin表达,在临床有效组（临床完全缓解和临床部分缓解组）差异有统计学意义（P〈0.05）,而临床稳定和进展组差异不显著（P〉0.05）。结论子宫动脉灌注化疗是Ⅱ期局部晚期宫颈癌有效治疗的手段之一,Survivin蛋白明显下调表明治疗有效,因此检测Survivin蛋白治疗前后的下调情况可成为局部晚期宫颈癌子宫动脉灌注化疗疗效评价的一个参考指标。Objective To evaluate the effects and clinical significance of survivin expression in intra-uterine arterial neoadjuvant chemotherapy for stage Ⅱ locally advanced cervical cancer.Methods Seventy-seven patients with locally advanced cervical cancer,FIGO Stage Ⅱ,from January 2005～December 2007 were enrolled in this study,those patients were primary treatment by neoadjuvant intra-uterine arterial chemotherapy.Immunohistochemistry were performed to eximine the expression level and quantity of surviving of pretherapy and posttherapy.Results The overall effective rate of the neoadjuvant chemotherapy for the 77 patients was 64.93%,and 61.11% for stage Ⅱa,66.10% for stage Ⅱb.The expression level of survivin on pretherapy and posttherapy were 63.64% and 44.15%,the differences between those showed significant（P0.05）.The expression of survivin on pretherapy and posttherapy groups showed significant（P0.05） in clinical remission groups（complete clinical remission and partial remission）,and no significant in progression or stable groups.Conclusion Intra-uterine arterial neoadjuvant chemotherapy is one of the effective means of treatment methods for locally advanced cervical cancer,survivin reducing significantly showed effective treatment,so survivin could be an indicator for locally advanced cervical cancer of intra-uterine arterial neoadjuvant chemotherapy sensitivity.
