局部晚期子宫颈癌动脉灌注化疗疗效分析

摘　　要：目的探讨动脉灌注化疗在Ⅱ期局部晚期宫颈癌治疗中的可行性及疗效。方法选择2005年9月~2008年6月在新疆医科大学附属肿瘤医院住院治疗的Ⅱ期局部晚期宫颈癌行新辅助动脉插管化疗的116例患者,分析其临床病理资料,介入化疗后86例行宫颈癌根治术,选择同期单纯手术治疗的Ⅱ期非局部晚期宫颈癌患者80例为对照组,比较介入手术组与单纯手术治疗组患者的病理组织学特点,术中,术后情况。结果介入化疗后宫颈局部及宫旁情况均有不同程度的改善,Ⅱ期宫颈癌患者近期有效率为74.14%,组织学完全缓解率为4.65%。介入化疗后手术组与对照组患者比较,病理高危因素、手术时间、术中出血量无差异（P〉0.05）;介入化疗后手术组术后并发症的发生低于单纯手术组,两组术后并发症发生有统计学差异（P〈0.05）。结论动脉灌注化疗可有效缩小局部晚期宫颈癌肿瘤体积和改善宫旁条件,提高手术切除率,减少术后并发症的发生。Objective To study the curative effects and feasibility of intra uterine arterial neoadjuvant chemotherapy in patients for locally advanced cervical cancer.Methods A retrospective study of 116 patients with FIGO stage Ⅱa～Ⅱb cervical cancer were performed intra-arterial neoadjuvant chemotherapy during the period of September 2005 and June 2008 in Affiliated Tumour Hospital,Xinjiang Medical University,after 2～3 weeks,86 patients followed radical operation and 30 patients followed radiotherapy.The study group（n=86） underwent surgery following intra uterine arterial neoadjuvant chemotherapy while control group（n=80） were selected among the patients of cervical cancer performed operations without chemotherapy.Compared two groups of pathological risk factor,operation time,bleeding amounts and operative complication.Results After intra uterine arterial neoadjuvant chemotherapy,the tumor size reduced significantly,the rates of complete response plus partial response were 74.14%,pathological complete responses were found in 4 cases（4.65%）;there were no significant differences in operation bleeding amounts,operation time and pathological risk factor between two groups.Compared with the control group,incidence of operative complications were statistically lower in experiment group（P0.05）.Conclusion Intra-arterial neoadjuvant chemotherapy is a safe and effective therapeutic option which could decrease local tumor size and infiltration of lateral uterus,reduce clinical stage and increase the radical resection rate,while no increasing of operative complications,operative time and bleeding amounts.
