摘  要:目的探讨皮质下缺血性血管性痴呆(SIVD)的病理生理特点。方法纳入45例皮质下缺血性血管性痴呆患者及51例作为对照的无痴呆型脑梗死患者,使用磁共振扫描仪对纳入对象进行扫描,比较其皮质下病灶部位、梗死灶大小及数目。结果与对照组比较,SIVD患者梗死病灶的分布在基底节区内囊前肢、尾状核,以及额叶、半卵圆中心丘脑有显著差异(P<0.05或P<0.01),SIVD患者基底节区内囊前肢、尾状核、豆状核及半卵圆中心后部、丘脑梗死灶数目较多(P<0.05或P<0.01),SIVD患者在颞叶、侧脑室体旁白质前部、半卵圆中心丘脑及前部区域梗死病灶体积增加较为显著(P<0.05或P<0.01)。结论皮质下梗死灶的部位、数目、体积与皮质下血管性痴呆的发生有着密切的关联。Objective To investigate the characteristics of MRI imaging of subcortical ischemic vascular dementia(SIVD).Methods 45 patients with SIVD(SIVD group) and 51 patients with non-dementia cerebral infarction(control group) were included in this study.Craniocerebral scanning was performed in both groups using a GE0.5T Signa profile MRI scanner.The site,size and number of subcortical infarction focuses were compared between the two groups.Results Compared with control group,anterior limb of internal capsule and caudate nucleus in basal ganglia,frontal lobe and thalamus in centrum ovale majus showed significantly different distribution of infarction focuses in SIVD group(P0.05 or P0.01).SIVD group had more infarction focuses in anterior limb of capsula interna,caudate nucleus and lentiform nucleus in basal ganglia,posterior part and thalamus in centrum ovale majus than control group(P0.05 or P0.01).Moreover,the size of infarction focuses in temporal lobe,white matter beside body of lateral ventricle,thalamus and anterior part in centrum ovale majus was bigger in SIVD group than that in control group(P0.05 or P0.01).Conclusion The site,number and size of subcortical infarction focuses may be related to the pathogenesis of SIVD.
