急性白血病患者血清及唾液中癌胚抗原测定的临床意义

摘　　要：目的观察急性白血病患者血清、唾液中CEA水平的变化。方法应用化学发光免疫分析方法,对110例急性白血病患者和100名正常健康人血清与唾液中CEA同时进行测定。结果急性白血病患者组血清CEA水平明显高于正常对照组(P<0.01);在110例急性白血病患者中有68例为阳性(61.8%),在100例正常健康人中有1例为阳性(1%)。急淋和急非淋白血病患者血清中CEA水平相近(P>0.05)。急性白血病患者组与正常对照组唾液中的CEA水平明显高于血清中的CEA水平,但两组比较,唾液中的CEA水平无明显性差异(P>0.05)。36例急性白血病患者缓解前后血清中CEA水平有极显著性差异(P<0.01)。13例白血病患者复发后血清CEA浓度为(7.93±5.54)μg/L,其中11例呈阳性(84.6%)。结论急性白血病患者血清中CEA浓度随病情变化而出现相应的变化。检测急性白血病患者血清中CEA水平变化,可作为患者在治疗期间监测病情变化的一种较好手段。测定唾液中CEA对白血病的诊断意义不太大。Objective To observe the carcino-embryonic antigen（CEA） levels in serum and saliva of patients with acute leukemia.Methods The electrochemical luminescent technique was used to detect the CEA levels in the blood serum and saliva of 110 acute leukemia patients and 100 normal persons.Results The blood serum CEA level in the acute leukemia patient group was significantly higher than that of the normal control group.68 cases were found positive（61.8%）,and 1 case was positive in the 100 normal healthy people（1%）.The blood serum CEA level showed no significant difference between ALL and AML（P 0.05）.The saliva CEA level was significantly higher than the CEA level in the blood serum in the two groups.But the saliva CEA level showed no significant difference between the acute leukemia patients and normal control group（P 0.05）.The blood serum CEA level in 36 cases of acute leukemia patients were significantly different before and after remission（P0.01）.The blood serum CEA level in 13 cases was7.93 ± 5.54μg / L after relapse,and 11 cases of them were found positive（84.6%）.Conclusion The blood serum CEA level in acute leukemia patients changes with the state of illness.Detecting blood serum CEA levels of acute leukemia patients can be used to monitor the condition of patients during treatments.Saliva CEA determination is of no great significance to the diagnosis of leukemia.
