替吉奥联合草酸铂治疗晚期胃癌临床研究

摘　　要：目的观察替吉奥(S-1)联合草酸铂(L-OHP)治疗晚期胃癌的临床疗效及安全性。方法将64例晚期胃癌患者随机分成试验组和对照组。试验组患者采用替吉奥60 mg,早晚各服1次,第1～14天,草酸铂130 mg/m2,静脉滴注1 d;对照组患者给予氟尿嘧啶联合顺铂化疗方案,氟尿嘧啶750 mg/d,静脉滴注,第1～5天,顺铂40 mg/d,静脉滴注,第1～3天。21 d为1个周期,至少连用2个周期后评价疗效。结果试验组完全缓解(CR)1例,部分缓解(PR)17例,稳定(SD)12例,进展(PD)2例。有效率(RR)为56.3%。不良反应较轻,主要表现为Ⅰ～Ⅱ度恶心、呕吐、腹泻等胃肠道反应,手足综合征,末梢神经感觉异常及血液学毒性。结论替吉奥联合草酸铂治疗晚期胃癌具有较好的疗效,能显著提高患者的生活质量。Objective To evaluate the efficacy and side effects of the combination of S-1 and Oxaliplatin（L-OHP） in patients with advanced gastric carcinoma.Methods Sixty-four patients with advanced gastic cancer were divided into treatment group and control group.The treatment group was given S-1（60 mg/d,twice daily,d1～14） combined with Oxaliplatin（130 mg/m2,ivgttd1）.The control group was treated with 5-FU（750 mg/d,ivgtt d1～d5） plus cisplatin（40 mg/d,ivgtt d1～3）.The regime was repeated every 21 days for at least 2 consecutive cycles.Results The results of evaluation in the treatment group were as follow： 1 CR,17 PR,12 SD and 2 PD.The overall effective rate was 56.3%.Adverse effects were generally light Ⅰ～Ⅱ including nausea and vomiting,diarrhea,stomatitis,hand and foot syndrome,neurotic and leukopenia.Conclusion S-1 plus Oxaliplatin is a highly effective regimen for patients with advanced gastric carcinoma and can remarkably improve patients′ life quality.
