摘 要:目的调查深圳市新安街道劳务工社区医疗服务的费用,探讨其收支不平衡的原因,为完善劳务工社区医疗服务购买政策提供依据。方法整群抽样抽取深圳市新安街道的10家社区卫生服务中心,通过深圳市网上医保标准化信息系统对其最近1年和最近1个月的劳务工医疗门诊费用进行调查。结果 (1)10家社区卫生服务中心最近1年劳务工的就诊率为20.67%,亏损率为16.79%,次均费用为43.56元。(2)最近1个月劳务工的就诊率为24.10%,亏损率为43.73%,次均费用为45.78元。(3)为达收支平衡,以最近1年劳务工的就诊情况估算每人每月社区门诊的支付水平应为7.01元,以最近1个月劳务工的就诊情况估算每人每月支付水平应为8.62元,都远高于目前每人每月6.00元的支付标准。结论劳务工就诊率增高、筹资和支付水平太低是社区医疗服务机构亏损的主要原因;深圳市劳务工社区医疗服务购买可从提高筹资和支付水平、降低单次费用以及加强监管等方面着手以达致收支平衡。Objective To investigate the expenses of labor workers′ community medical service(LWCMS) in Xinan communities of Shenzhen City,and study the reason of imbalance between receipts and expenditure,in order to provide some basis for developing the governmental purchase policy of LWCMS.Methods Total 10 community health service centers(CHSC) were selected by using cluster random sampling for the survey of expense data of LWCMS in the last 1 year and 1 month by online medical insurance standardizing system of Shenzhen.Results(1)In the last 1 year the labor workers′ consultation rate in the 10 CHSC was 20.67%,the deficit rate was 16.79%,the average expense per time was RMB 43.56 yuan.(2)In the last 1 month the labor workers′ consultation rate in the 10 CHSC was 24.10%,the deficit rate was 43.73%,the average expense per time was RMB 45.78 yuan.(3)The labor workers′ expense was estimated to be RMB 7.01 yuan per capita a month according to the labor workers′ consultation condition in the last 1 year,and RMB 8.62 yuan in the last 1 month.The two numbers were much higher than the present standard of RMB 6.00 yuan for a balance between receipts and expenditure.Conclusion The main reasons for the deficit of CHSC are hither labor workers consultation rate,lower financing and payment levels.It is necessary for the Shenzhen government to balance in labor workers community medical service by raising finance and payment standard,reducing expense per time,and strengthening supervision.
