实验猪血流量及压力测定的麻醉管理

摘　　要：目的探讨实验猪血流量及压力测定的麻醉管理,评价该实验期间麻醉效果。方法巴马香猪10头(雄雌各半),体重35～40 kg,月龄5～6个月。术前12 h禁食,6 h禁水。麻醉前15 min在臀大肌肌肉注射氯胺酮10mg/kg,阿托品0.02 mg/kg,咪达唑仑0.1 mg/kg作基础麻醉。开放耳缘静脉,颈内静脉置F5双腔管,监测心电图、脉搏血氧饱和度(SPO2;可夹在舌头上),鼻咽温,股动脉置20号套管针监测动脉压。麻醉诱导氯胺酮10 mg/kg,速眠新Ⅱ0.1 mg/kg联合静脉注射,保留呼吸行气管插管,压力控制呼吸,潮气量8～12 ml/kg,空氧混合气体吸入。麻醉维持速眠新Ⅱ0.1 mg/kg追加1次/60 min,辅助异氟醚0.8%～1.4%吸入。实验方法:实验猪取仰卧位,胸骨正中切口,前腔静脉近心端、远心端,主肺动脉瓣上1 cm,左、右肺动脉起始部,缝制荷包线,插套管针连接压力感受器、测量血管内压力,测压完成后,应用Veri Q 4221流量仪,测量上述部位血管流量。结果实验中基础麻醉效果较好,气管插管一次成功,无麻醉死亡;麻醉维持稳定,血流动力学平稳,术中无疼痛反射,肌肉松弛良好,总时间2.5～3.0 h,实验结束后均在10～15 min内恢复呼吸,一次拔除气管导管,无感染,24 h进食,放养于猪舍内至今全部存活。结论麻醉处理合理,效果良好,麻醉方法可行性强,用药合理,保障实验顺利进行。Objective To evaluate the anesthesia effect of the experiment pigs for blood flow and pressure determination.Methods The 10 Bama miniature pigs,including 5 males and 5 females,weight 35 to 40 kg,aged from 5 to 6 months,were chosed in the test.Before the operation,the pigs were dealed with 12 hours fasting and 6 hours water forbidding.Ketamine 10 mg/kg,atropine 0.02 mg/kg and midazolam 0.1 mg/kg were given by intramuscular 15 min before the operation for basic anesthesia.Auricular margin veins were opened and F5 dual-lumen indwelling catheter was placed in internal jugular.The ECG,SPO2 and the nasopharyngeal temperature were measured.The BP was measured by femoral artery puncture with 20# trocar.Ketamine 10 mg/kg combined with SumianxinⅡ0.1 mg/kg was used by vein for induction of general anesthesia.The tracheal intubation was carried on reserving breathing of the pigs.The mixed gas of air and oxygen was applied and the PCV mode was adopted in mechanic ventilation and the VT was set in 8-12 ml/kg.SumianxinⅡ0.1 mg/kg was applied per 60min and 0.8-1.4% isoflurane inhalation in necessary for anesthesia maintenance.Test methods： The test pigs were disposed to supine position and were opened with median incision on Sternum.Purse-string suture was stitched on pericenter and apocenter of first cavity vein,1 cm away from main pulmonary valve and the initiation part of left and right pulmonary atery,and then the trocar was placed.The pressure sensor was connected with trocar to monitor the blood pressure and the blood flow of above parts was measured by VeirQ4221.Results The effect of basic anesthesia for test pigs was favourable and there was no death induced by anesthesia.The anesthesia maintenance and the hemodynamics were stable.The surgery time was 2.5-3 hours.There was no pain reflex and the effect of muscle relaxation was satisfactory.The respiratory of all pigs recovered with in 10-15 min after the tests and the tracheal intubation was extracted.All the pigs feeded 24 hours after the tests.No inflammation occu
