摘　　要：目的通过对短暂性脑缺血发作(TIA)的多种危险因素的分析达到对TIA后脑梗死的预警及防治的目的。方法搜集2006年8月至2009年1月在太原市中心医院、临汾市人民医院住院,临床确诊的199例TIA患者的住院临床资料,进行90d的随访。结果 TIA后脑梗死是多种因素,多种机制共同作用的结果;症状持续时间≥1h、言语障碍、肢体无力、体质量指数(BMI)≥25kg/m2、心功能不全、既往颅内血管事件病史、CT/MRI显示缺血性卒中、颈动脉超声斑块这8个因素是TIA后90d发生脑梗死的危险因素。结论对于TIA进展至脑梗死的患者,14d发生脑梗死的危险因素为:肢体无力、心功能不全、既往颅内血管事件病史、颈动脉超声斑块。90d发生脑梗死的危险因素为:症状持续时间≥1h、言语障碍、肢体无力、BMI≥25kg/m2、心功能不全、既往颅内血管事件病史、CT/MRI显示缺血性卒中、颈动脉超声斑块。Objective Cerebral infarction （CI） is the common and severe final outcome after the transient ischemic attack （TIA） occurrce.This study has achieved the goals of warning and prevention of CI after TIA with multiple factor analysis of TIA occurrence.Methods Between August 2006 and January 2009,a total of 199 TIA patients clinically confirmed and hospitalized in Taiyuan Center Hospital and Linfen People＇s Hospital were recruited for their clinical datas and followed up for 90 days.Results CI occurrence after TIA results from the interaction of multi-factors and multiple mechanisms.Duration of symptoms≥1 h,speech disorders,limb weakness,BMI≥25 kg/m2,cardiac dysfunction,disease history of intracranial vascular events,ischemic stroke on CT/MRI,carotid ultrasound plaque are eight risk factors of CI occurrence at 90 days after TIA.Conclusion This study reveals the risk factors of CI occurrence due to 14 days of TIA progression,including limbs weakness,cardiac dysfunction,disease history of intracranial vascular events and carotid ultrasound plaque.While the risk factors of CI at 90 days after TIA are duration of symptoms≥1 h,speech disorders,limb weakness,BMI ≥25 kg/m2,cardiac dysfunction,disease history of intracranial vascular events,ischemic stroke on CT/MRI and carotid ultrasound plaque.
