手辅助腹腔镜全胃切除术治疗巨大胃淋巴瘤

摘　　要：目的探索手辅助方式在腹腔镜胃癌D2根治术中应用的可行性和技术路径。方法设计手术技术路径,选择1例男性78岁胃恶性淋巴瘤患者,采用手辅助方式进行全胃切除术,同时切除部分肿大的淋巴结。结果全胃切除术和部分腹腔淋巴结切除术均取得了成功,组织切缘未见肿瘤残留,腹腔淋巴结未发现肿瘤转移。手术用时206min。患者术后第3天进食流质饮食,第7天出院。结论我们设计的手术路径完成了首例手辅助腹腔镜全胃切除术,改进后可适用于手辅助腹腔镜胃癌D2根治术。Objective To explore the feasibility of application of hand-assisted laparoscopic surgery for D2 radical gastrectomy.Methods A 78-year-old man with gastric malignant lymphoma unerwent a hand-assisted laparoscopic total gastrectomy.Part of the enlarge lymph node was scavenged.Results Total gastrectomy and celiac lymph node resection were done.The esected tissue was negative for residual tumor.And the celiac lymph nodes were free of metastasis.Operative time was 206 minutes.Conclusions This is the first case of hand-assisted laparoscopic total gastrectomy with a designed method.With the modified method,D2 radical gastrectomy may be more feasible.
