非小细胞肺癌放疗前后血清肿瘤标记物的改变及其临床意义

摘　　要：目的观察非小细胞肺癌(NSCLC)放疗前后血清CEA、CYFRA21-1、NSE的变化,探讨血清肿瘤标记物的改变能否用于评价放疗疗效。方法选取58例接受放射治疗的非小细胞肺癌病例(其中22例为局部晚期患者行胸部原发灶放疗,12例行骨转移放疗,24例行脑转移放疗),所选病例放疗前肿瘤标记物均高于正常参考值,分别记录放疗前与放疗后血清CEA、NSE、CYFRA21-1的数值,并用Wilcoxon秩检验进行统计分析,以p<0.05为有统计学意义。结果 22例胸部放疗患者放疗后血清CEA、NSE、CYFRA21-1水平较放疗前降低(P值分别为0.046,0.003,0.002),12例骨转移患者放疗后血清CEA、NSE、CYFRA21-1水平较放疗前降低(P值分别为0.05,0.015,0.017),24例肺癌脑转移患者放疗前后血清CEA、NSE、CYFRA21-1水平无统计学差异(P>0.05)。结论血清CEA、CYFRA21-1、NSE水平可以用于非小细胞肺癌胸部放疗及骨转移放疗后的疗效评价。Objective To observe the impact of radiotherapy on serum tumor marker such as CEA,NSE,CYFRA21-1 in patients with non-small cell lung cancer（NSCLC）,Investigate whether the change of tumor maker can evaluate the effect of radiotherapy.Methods Fifty-eight NSCLC patients were treated with radiotherapy and tested serum TM（CEA,NSE,CYFRA21-1） before and after radiotherapy,tumor marker of all the cases were higher than normal reference values,record the value of TM before and after radiotherapy respectively.Results The value of serum TM（CEA,NSE,CYFRA21-1） was deceased of twenty-two patients with lung primary tumor and twelve patients with bone metastasis,there was no significant difference in serum TM（CEA,NSE,CYFRA21-1） of twenty-two brain metastasis before and after radiotherapy.Conclusion The value of serum TM（CEA,NSE,CYFRA21-1） can evaluate the effect of radiotherapy with lung primary tumor and bone metastasis patients.
