摘 要:目的探讨抑制枯否细胞(Kupffer cells,KCs)对脓毒症大鼠肝脏微循环的影响。方法采用盲肠结扎穿孔法(CLP)制备大鼠脓毒症急性肝损伤模型。造模前1d和2d经尾静脉注射三氯化钆(GdCl3)去除KCs。将60只健康SD大鼠随机分为四组,即对照组、假手术组(Sham组)、模型组(CLP组)和GdCl3组(CLP+GdCl3组)。各组动物于术后24h处死。检测血中丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、内毒素(ET)、内皮素-1(ET-1)及一氧化氮(NO)的水平。提取肝组织RNA,采用RT~PCR方法检测ET—1、eNOS、INOS及HO—1mRNA的表达。结果与对照组比较,脓毒症大鼠血中ALT、AST、ET、ET-1及NO水平显著升高(P〈0.05);去除KCs后,脓毒症大鼠血中ALT、AST、ET、ET—1及NO水平显著降低(P〈0.05)。脓毒症大鼠肝组织ET-1、eNOS、iNOS及HO-1mRNA表达水平较对照组显著升高(P〈0.05);去除KCs后,肝组织中iNOS及HO-1mRNA表达水平显著降低(P〈0.05)。结论KCs在脓毒症时肝脏的微循环障碍中起重要作用。Objective To investigate the influence of Kupffer cells (KCs) inhibition on liver microcirculation in the rats with sepsis. Methods The sepsis model of Sprague - Dawley (SD) rat by cecal ligation and puncture (CLP) was used. The KCs were inactivated by gadolinium chloride ( GdCl3 ) at 1 and 2 d before operation via tail vein. Sixty healthy male SD rats were randomly divided into 4 groups:control group, sham operation group (Sham group), sepsis group (CLP group)and GdCl3 group (CLP + GdC13 group). All rats were sacrificed at 24 h after operation. The blood was collected to detect the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), endotoxin (ET), endothelin - 1 ( ET - 1 ) and nitrogen monoxide (NO). The liver was harvested to analyze the messenger RNA (mRNA) expressions of ET - 1, eNOS, iNOS and HO - 1 by reverse transcriptase polymerase chain reaction ( RT - PCR). Results Compared with control group, the levels of ALT, AST, ET, ET - 1 and NO in blood were significantly increased in CLP group ( P 〈 0.05 ). The increases were suppressed after the inhibition of KCs by GdCI3. Moreover, compared with control group, the mRNA expressions of ET - 1, eNOS, iNOS anti HO - 1 were significantly increased after CLP, This increases in the mRNA expression of iNOS and HO - 1 were attenuated by GdCls ( P 〈 0.05 ). Conclusion The KCs may play an important role in the disturbance of liver mierocirculation in sepsis.
