89例老年晚期非小细胞肺癌患者化疗方案的临床分析

摘　　要：目的探讨老年晚期非小细胞肺癌(NSCLC)患者的化疗方案。方法采用回顾性研究方法分析89例接受化疗的老年晚期NSCLC患者,将其按年龄分为高龄老年组(≥75岁)与老年组(≥70～75岁),按化疗方案分为单药化疗组、不含铂联合化疗组及含铂联合化疗组。比较各组的疗效、生活质量、生存期及不良反应。结果 89例老年晚期NSCLC患者化疗总有效率为31.5%(28/89),中位生存期9.9个月;Ⅲ～Ⅳ级骨髓抑制和Ⅲ～Ⅳ级消化道反应发生率分别为30.3%和6.7%,治疗期间均未发生因化疗不良反应所致死亡。各组在化疗疗效、生活质量、生存期及不良反应方面差异均无统计学意义(P>0.05)。结论化疗对老年晚期NSCLC患者疗效好,单药化疗、含铂或不含铂联合化疗疗效相似。Objective To study the therapeutic regimen in treatment of elderly patients with non-small lung cancer（NSCLC） in advanced stage.Methods A retrospective study was conducted to analyze 89 elderly patients with advanced NSCLC who received chemotherapeutic treatment.They were divided into two groups according to their ages：senile group（≥75 years old） and pre-senile group（≥70-75 years old）.They were also divided into three groups according to chemotherapeutic regimen：single-agent group,non-cisplatin combined group and cisplatin combined group.Effectiveness,life quality and adverse reaction of different patient groups were compared.Results The response rate for all 89 elderly patients with advance NSCLC was 31.5%（28/89）.The median survival time was 9.9 months.Incidence rates of grade Ⅲ-Ⅳ myelosuppression and grade Ⅲ-Ⅳ gastrointestinal toxic reaction were 30.3% and 6.7%,respectively.No chemotherapeutic toxic effect related death had been observed during treatment.There was no statistically significant difference（P0.05） in the effectiveness of chemotherapeutic treatment,life quality,survival time and toxic reaction among different patient groups.Conclusion Chemotherapy shows good effects in treatment of elderly advanced NSCLC patients,and therapeutic effects of single-agent,non-cisplatin combined and cisplatin combined group are similar.
