摘 要:目的探讨胃癌中蛋白酪氨酸磷酸酶非受体型11(PTPN11)基因多态性与幽门螺杆菌(Hp)感染的相关性。方法选取我院胃镜检查诊断为胃癌的238例及慢性胃炎112例患者为研究对象。收集两组患者的胃黏膜标本,通过PCR检测Hp的尿素酶B亚单位(UreB)基因;两步法聚合酶链反应(PCR-CTPP)对PTPN11基因第3内含子2460位点进行单链构象多态性分析(SSCP)。并比较这些基因多态性在胃癌组和对照组的分布差异。结果胃癌组Hp阳性者为142例(59.66%),阴性者96例(40.34%);对照组Hp阳性者54例(48.21%),阴性者58例(51.79%),Hp感染率在两组间差异有显著性(P〈0.05)。胃癌组和对照组PTPN11的等位基因分布差异有显著性(P〈0.05)。将G/A型和A/A型合并后与G/G型相比,G/G基因型的个体患胃癌的风险增加(OR=0.620,95%CI:0.388~0.992)。G/G基因型合并Hp阳性的个体相对于A基因携带者患胃癌的风险增加到0.52倍(OR=0.521,95%CI:0.274~0.990)。结论 PTPN11基因第3内含子2460位点G/G基因型增加胃癌的发病风险,Hp感染与该位点G/G基因型之间存在交互作用。Objective To investigate the association between protein-tyrosine phosphatase,non receptor-type 11(PTPN11) gene polymorphisms and Helicobacter pylori(Hp) infection in patients with gastric cancer. Methods 238 patients with gastric cancer proven by gastroscopy and 112 patients with chronic gastritis were selected in this study.Samples of gastric mucosa in these two groups were collected.Urease B subunit(UreB) gene of Hp was detected using PCR test.In PTPN11 gene polymorphism at position 2460 in intron 3,the single-strand conformation polymorphism(SSCP) was analysed using the polymerase chain reaction with confronting two-pair primers(PCR-CTPP).We compared the distribution difference of gene polymorphisms between gastic cancer group and normal control group.Results In gastric cancer group,142 patients(59.66%) carried with possitive Hp,96 patients(40.34%) carried with negative Hp;in control group,54 patients(48.21%) carried with possitive Hp and 58 patients(51.79%) carried with negative Hp.There was statistically significant difference of Hp infected rate between these two groups(P0.05).There was statistical significant difference of PTPN11 allelic distribution between gastric group and control group(P0.05). The individuals with G/G genotype significantly increased gastric carcinoma risk(OR=0.620,95%CI: 0.388~0.992) when compared with G/A genotype in combination with A/A genotypes.Compared to A gene carriers,individuals with G/G genotype in combination with positive Hp increased the risk of gastric cancer to 0.52 times(OR =0.521,95%CI: 0.274~0.990).Conclusion The presence of G/G genotype at the site of 2460 in intron 3 of PTPN11 increased the risk of gastric cancer,G/G genotype at the site might contribute to interaction with Hp infection.
