套筒冠与高强纤维黏结在固定重度牙周炎松动牙中的比较研究

摘　　要：目的比较套筒联冠修复方式与高强纤维黏结方式对重度牙周炎松动牙齿固定的临床效果。方法选取20例重度牙周炎的患者,86颗患牙,按随机数字表法分为套筒冠组和高强度纤维组,每组各10例,其中高强纤维黏接组需进行夹板修复的松动牙46颗;套筒冠组需进行夹板修复的松动牙40颗。在系统牙周治疗和根管治疗后,对松动牙制作套筒连冠和高强纤维黏结固定,保持每年2次牙周洁治,检查记录基牙探诊深度(probing depthPD)、龈沟出血指数(sulcus bleedingindex SBI)。结果经过修复前及修复后6个月、1年、2年的观察,套筒冠组龈沟探诊深度分别为(4.38±0.52)、(3.32±0.34)、(2.62±0.37)、(2.58±0.41)mm,龈沟出血指数分别为1.32±0.21、1.22±0.41、1.34±0.37、1.35±0.41;高强纤维组龈沟探诊深度分别为4.26±0.46、3.44±0.32、2.76±0.39、2.75±0.79 mm,龈沟出血指数分别为1.46±0.27、1.49±0.88、1.69±0.49、1.75±0.83;2组修复后龈沟深度皆有减小,龈沟出血指数高强纤维黏结固定组修复1年后较套筒冠组为高(P<0.05)。结论套筒冠和高强纤维黏结固定夹板都对维持牙周健康有利,套筒冠固定夹板修复更有利于牙周卫生的保持。Objective To compare the clinical effects of telescopic crown and fibre reinforced composites for splinting loose teeth after periodontal treatment.Methods Twenty patients with severe periodontitis（86 teeth） were randomly divided into telescopic crown group（40 teeth） and fibre-reinforced composites group（46 teeth）.After periodontal and root canal treatment,loose teeth were fixed by telescopic crown and fibre reinforced composites.Periodontal scaling was performed twice a year,and probing depth（PD） and gingival sulcus bleeding index（SBI） were measured.Results Before treatment and 6 months,1 year and 2 years after treatment,PD was（4.38±0.52） mm,（3.32±0.34） mm,（2.62±0.37） mm and（2.58±0.41） mm in telescopic crown group,respectively,and（4.26±0.46） mm,（3.44±0.32） mm,（2.76±0.39） mm and（2.75±0.79） mm in fibre-reinforced composites group,respectively.SBI was 1.32±0.21,1.22±0.41,1.34±0.37 and 1.35±0.41 in telescopic crown group,respectively,and 1.46±0.27,1.49±0.88,1.69±0.49,and 1.75±0.83 in fibre-reinforced composites group,respectively.The depth of gingival sulcus decreased in both groups,and SBI in fibre-reinforced composites group was higher than that in telescopic crown group 1 year after treatment（P0.05）.Conclusion Compared with fibre reinforced composites,telescopic crown is more conducive to the maintenance of periodontal health.
