超声测量肾周及肾旁脂肪厚度在评估非酒精性脂肪肝患者肝脂肪浸润程度中的意义

摘　　要：目的探讨腹部超声测量肾周及肾旁脂肪厚度(UFT)在定量评估非酒精性脂肪肝(NAFLD)患者肝脂肪浸润程度中的意义。方法应用彩色多普勒超声测量90例NAFLD患者肾周及肾旁脂肪厚度(UFT)、腰围及体质量指数(BM I)。根据肝脂肪浸润程度将NAFLD患者分为轻度、中度和重度脂肪肝组。结果轻度、中度和重度NAFLD患者肾周及肾旁UFT比较,差异有统计学意义(P<0.05)。NAFLD患者肾周及肾旁UFT和肝脂肪浸润程度比较,呈显著正相关(r=0.89,P<0.01)。NAFLD患者肾周及肾旁UFT和腰围比较,呈显著正相关(r=0.81,P<0.01)。NAFLD患者肾周及肾旁UFT和BM I比较,呈显著正相关(r=0.79,P<0.01)。结论超声测量肾周及肾旁UFT在预测NAFLD患者脂肪肝的发病和发展中有重要的临床应用价值,可准确地评估NAFLD患者肝脂肪浸润程度。Objective To explore the value of ultrasound measurement of peri-and para renal fat thickness （UFT） in assessment of the degree of liver fatty infiltration in patients with nonalcoholic fatty liver disease （NAFLD）. Methods Ninety patients with NAFLD were enrolled in this study. Their peri-and para renal UFT, waist circumference and body mass index （BMI） were measured by ultrasonography. According to the degree of liver fatty infiltration, the patients were divided into mild group, moderate group and severe group. Results There was significant difference of peri-and para renal UFT between mild, moderate and severe group （P 〈 0.05 ）. There was significant positive correlation between peri-and para renal UFT and liver fatty infiltration in patients with NAFLD（r =0. 89, P 〈0.01 ）. There was significant positive correlation between peri-and para renal UFF and waist circumference in patients with NAFLD（r =0. 81, P 〈 O. 01 ）. There was significant positive correlation between periand para renal UFT and BMI in patients with NAFLD （ r = O. 79, P 〈 O. 01 ）. Conclusion Ultrasound measurement of peri- and para renal UFT has important clinical value in oceuranee and development of liver fatty in patients with NAFLD, it can accurately assess the degree of liver fatty infiltration in patients with NAFLD.
