补血益气组方联合心理干预治疗产后抑郁症的临床研究

摘　　要：目的观察补血益气组方联合心理干预对产后抑郁症的疗效。方法选择2007年8月至2010年3月经爱丁堡产褥期抑郁症评定量表(EPDS)确诊的产后抑郁症患者49例,完全随机分为心理干预组24例,心理干预联合中药治疗组(联合组)25例。两组均给予产后一般护理,心理干预组在给予产后护理基础上,由专人进行规范化的心理干预;联合组在一般护理和心理干预的基础上口服补血益气组方。两组均于住院分娩前及产后7 d、42 d、3个月采用EPDS进行评估,并对结果进行分析。结果心理干预组分娩前及产后7 d、42 d、3个月EPDS评分为(16.14±2.95)分、(13.67±2.85)分、(11.98±2.66)分、(8.78±2.11)分;联合组分别为(16.06±2.90)分、(12.37±1.85)分、(8.62±2.32)分、(6.78±2.03)分。两组产后7 d、42 d、3个月EPDS评分较产前减分值均有减少,心理干预组与联合组产后7 d、42 d、3个月EPDS评分与产前比较,差异有统计学意义(P<0.05或P<0.01),联合组产后42 d、3个月EPDS评分与心理干预组比较,差异有统计学意义(P<0.01)。结论补血益气组方联合心理干预能明显降低抑郁症患者EPDS评分,改善抑郁焦虑症状。Objective To observe the effect of Buxueyiqi Chinese medicine combined with psychotherapy on postpartum depression.Methods Total 49 patients of postpartum depression,based on Edinburgh postpartum depression rating scale（EPDS）,were enrolled and divided randomly into 2 groups：psychotherapy group（group A,24 cases,conventional therapy plus psychotherapy）and Buxueyiqi Chinese medicine with psychotherapy group（group B,25 cases,conventional therapy plus psychotherapy and Buxueyiqi Chinese medicine）.EPDS was applied to evaluate the clinical data of all 2 groups before and 7 d,42 d,3months after labor.Results EPDS decreased at 7 d,42 d,3 months after labor among the two groups,including significant changes in group A and group B（P0.05 or P0.01）.Compared with group A,EPDS showed significant differences at 42 days and 3 months after labor in group B.Conclusion Buxueyiqi Chinese medicine combined with psychotherapy can reduce the EPDS,improve symptoms and quality of life among patients with postpartum depression.
