激素联合丙种球蛋白治疗多发性硬化的临床研究

摘　　要：目的探讨激素联合丙种球蛋白治疗多发性硬化(MS)的疗效。方法随机将41例多发性硬化患者分为激素联合丙球治疗组(20例,联合组)和单纯应用激素治疗组(对照组,21例)。两组均用激素治疗,联合组另给丙种球蛋白0.4 g/kg,每日1次静脉注射,连续治疗5 d。所有患者在治疗前和开始治疗后的3周,均检测临床及免疫学指标。结果联合组治疗后,EDSS评分与治疗前比较,差异有统计学意义(P<0.01)。联合组治疗后与同组治疗前、对照组治疗后比较,IFN-γ的水平差异具有统计学意义(P<0.05),联合组治疗后与同组治疗前、对照组治疗后比较,IL-6的水平差异具有统计学意义(P<0.05);而治疗后联合组IL-10的水平明显高于对照组(P<0.05)。结论激素联合丙种球蛋白治疗多发性硬化,可通过非特异性的免疫调节机制调节机体的免疫功能,对多发性硬化的异常免疫应答起调节作用,可有效改善患者症状。Objective To explore the effect of immunoglobulin with hormone treatment for multiple sclerosis.Methods 41 cases of MS patients were divide into 2 groups：treatment group with immunoglobulin plus hormone（20 cases）and control group with hormone alone（21cases）.Each group received hormone while treatment group with immunoglobulin plus hormone received 0.4 g/kg immunoglobulin every day for 5 d intravenously.All clinical and immunological variables were measured at the baseline and 3 weeks after hormone therapy.Results There was significant difference between the treatment group and the control group in the expanded disability status scale scores（P0.05）.The levels of IFN-γ and IL-6 in treatment group were significantly lower than those of the control group and the treatment group before treatment（P0.05）.The levels of interleukin-10 in treatment group also were significantly higher than those of control group（P0.05）.Conclusion Immunoglobulin with hormone treatment for multiple sclerosis can adjust the body′s immune function by nonspecific immune regulation mechanism,help regulate the abnormal immune response of MS,more effectively improve the symptoms of MS.
