乳腺癌钙化在全数字化乳腺摄影中的特征分析

摘　　要：目的探讨全数字化乳腺摄影(Full-field digital mammography,FFDM)中的乳腺癌钙化的影像特征,以提高乳腺癌的诊断水平。方法收集经手术及病理证实的216例乳腺癌钙化病例,术前均行FFDM检查,常规采用头尾位、内外斜位,必要时加照侧位及局部点片。结果 216例不同病理类型的乳腺癌的FFDM检查影像特征不同,其中单纯钙化52例,肿块伴钙化138例,结构扭曲伴钙化26例。结论钙化多以细小多形性钙化,线样、针尖状、泥沙样钙化及簇状钙化为典型表现。综合分析钙化的影像特征,对乳腺癌有重要的诊断价值,并根据钙化的分布可提示乳腺癌的病理类型。Objective To explore the imaging features of breast carcinoma calcification on Full-field Digital Mammography（FFDM）,and to improve the diagnostic level of breast carcinoma.Methods 216 cases of breast carcinoma calcification confirmed by surgery and pathology were collected in this series.Preoperative FFDM examination was performed,craniocaudal（CC） view and mediolateral oblique（MLO） view were conducted for each patient.Mediolateral（ML） and spot view were achieved if necessary.Results 216 patients with different pathological characteristics of breast carcinoma were different on imaging feature in FFDM examination,of all the cases,solitary calcification was found in 52 cases,mass associated with calcification in 138 cases,structure distortion associated with calcification in 26 cases.Conclusion Most typical calcifications were minute calcification,line-like,needle-like,sand-like and clustered calcification.Comprehensive analysis of the calcification imaging features was important for the diagnosis of breast carcinoma,and may indicate the pathological type of breast carcinoma according to the distribution of calcification.
