色素放大内镜与普通电子肠镜靛胭脂染色诊断结肠粘膜肿瘤样病变的价值比较

摘　　要：目的比较普通电子结肠镜靛胭脂染色与色素放大内镜（FICE系统）对结肠粘膜肿瘤样病变的检出率及操作用时，分析FICE系统在临床诊断中的使用价值。方法对2006年10月-2010年9月期间随机行普通电子结肠镜检查与FICE系统检查的病例资料进行回顾性分析．前者自降乙交界处开始行靛胭脂染色，两者均从此处开始计时并记录结果，以病理组织学诊断为标准，比较两种检查对肿瘤样病变检出的灵敏度和操作用时。结果与病理组织学诊断比较。普通电子结肠镜靛胭脂染色诊断结肠粘膜肿瘤性和非肿瘤性病变的灵敏度、特异度、阳性预测值和阴性预测值依次为70．0％、96．0％、77．8％、94．1％，平均操作用时为5．41±O．58分钟；FICE系统的灵敏度、特异度、阳性预测值和阴性预测值依次为75．0％、95．8％、81．8％、93．9％，平均操作用时为3．54±0．43分钟，其中灵敏度及阳性预测值均有所提高，但两者比较不具有统计学意义（P=1．000）；而FICE系统检查较普通电子结肠镜检查用时缩短，两者比较具有统计学意义（P=0．000）。结论FICE系统较普通电子结肠镜靛胭脂染色对结肠肿瘤的检出具有更加良好的临床应用价值。Objective To compare with normal electronic colonoscopy and pigment magnifying colonoscopy （FICE system） on the detection rates and operate time for tumor-like lesion of colonic mucosa, and analysis the valuation of pigment magnifying colonoscopy in the clinical diagnosis. Methods Retrospective analysis on the inspection results with normal electronic colonoscopy and normal electronic colonoscopy during October 2006 and September 2010 with random, and compare the two kinds of checks on positive rate and accuracy of detection for tumors, pathology histological diagnosis as the standard. At the same time, we compare with the operate time. Results The sensitivity, specificity, positive predictive value and negative predictive value of normal electronic colonoscopy and FICE system compared with histological diagnosis of tumor lesions and non-tumor lesions were70.0%, 96.0%, 77.8% and 94.1%, respectively; 75.0%, 95.8%, 81.8% and 93.9%, respectively. The operate time were 5.41±0.58 minutes of normal electronic colonoscopy and 3.54±0.43 minutes of FICE system. Conclusion Detection rate of colon minimal tumor with pigment magnifying colonoscopy has improved significantly compare to normal electronic colonoscopy.
