摘  要:目的 通过观察涎腺腺样囊性癌 (adenoidcysticcarcinoma ,ACC)中基膜样物质的形态学改变 ,了解该瘤的浸润和血行转移的机制。方法 对 8例ACC的基膜样物质与 2例腮腺腺泡及闰管的基膜进行光镜和电镜观察。结果 电镜观察基膜样物质在肿瘤团索周围显示 :多层、溶解或断裂现象。瘤细胞伸出伪足样突起 ,且胞浆内含大量微泡 ,该处基膜消失 ,胶原原纤维溶解 ,肿瘤细胞围绕血管部位的血管基膜增厚 ,纤维组织增生。当癌细胞贴近血管时 ,胶原溶解 ,基膜消失。结论 ACC浸润生长是由于肿瘤细胞释放某种消化酶 ,使瘤周的基膜样物质和胶原原纤维溶解 ,并侵入血管内 ,发生血行转移。Objective To study morphological variations of basment membrane component (BMC) for detemination of invasion and vascular metastasis in adenoid cystic carcinoma (ACC). Methods The BMC of 8 cases in ACC and 2 cases of normal parotid gland were observed by LM and EM. Results The BMC surrounding tumor mass and strands showed multi layered or dissolution, pseudopodia were seen extended from tumor cells, and contained numerous microvesicles. If the tumor cell attached blood vessels, the collagenous fibril were dissoluted and BMC were lost. Conclusion The tumor cell may release some digestive enzymes which may be able to dissolve BMC and collagenous fibrils and invade into blood vessels and matastasis take place.
