摘　　要：目的探讨非手术因素对颞叶癫癎病人手术治疗的影响及应对策略。方法对10个月门诊筛查的67例颞叶癫癎病人进行问卷式调查,针对颞叶癫癎病人手术治疗的非手术因素进行调查分析。结果癫癎病史2~26年,平均8.3年,非正规治疗史43例（64.1%）。其他医院诊断为颞叶癫仅24例（35.8%）。知道手术可治疗癫32例（47.8%）,其中由医生告知可行外科手术11例（16.4%）,通过媒体和病友介绍21例（31.3%）。不愿接受手术治疗20例（30.0%）,其中因经济原因13例,害怕手术7例。结论病人及基层医师对颞叶癫癎及其正确治疗的认识不够是造成病人延误治疗的主要因素,病人心理因素和经济拮据也是重要原因。加强癫癎科普宣传,特别是推广《癫癎诊疗指南》有助于解决这些问题。Objective To investigate influence of non-surgical factors on surgical treatment of temporal lobe epilepsy and countermeasures.Methods Sixty-seven patients with temporal lobe epilepsy screening from outpatient clinic during 10 months were investigated with questionnaires in terms of non-surgical factors in the surgical treatment of temporal lobe epilepsy patients.Results The history of epilepsy ranged from 2 to 26 years with a mean period of 8.3 years.Forty three patients（64.1%） had received the informal treatments.Only 24 patients（35.8%） were diagnosed as having temporal lobe epilepsy in other hospitals.There were 32（47.8%） patients who heard about the surgical treatment for epilepsy,of whom 11（16.4%） got it from doctors and 21（31.3%） from the media and other patients.Twenty patients（30.0%） rejected the surgical treatment mainly for economical difficulty in 13 and fearing operation in 7.Conclusions Inadequate understanding of the patients and doctors about temporal lobe epilepsy and its correct treatments is a main factor for delay of treatment,and psychological and economical factors of the patient are also important reasons.To strengthen health education and especially popularization of the guideline for diagnosis and treatment of epilepsy helps to solve these problems.
