头颈肿瘤患者放疗前牙面用氟化钠涂膜预防放射性龋齿临床研究

摘　　要：目的观察牙面使用氟化钠涂膜对接受放射治疗的头颈部肿瘤患者龋齿病患病的影响,探讨氟化钠涂膜对放射性龋齿的预防作用。方法选择头颈部肿瘤进行放射治疗患者35例,用自身对照方法分为氟化钠涂膜组和涂生理盐水组。患者于放射治疗前1、3、5 d将氟化钠涂膜涂于牙面（氟化钠涂膜组）,将生理盐水涂于牙面（生理盐水组）,于放射治疗前和放射治疗后18个月分别观察两组患者的龋失补牙数（DMFT）和龋失补牙面数（DMFS）。结果患者放射治疗前,氟化钠涂膜组和生理盐水组DMFT和DMFS无统计学意义（P〉0.05）。与治疗前比较,放射治疗18个月后两组DMFT和DMFS均明显升高（P〈0.01）,但涂生理盐水组DMFT、DMFS升高更为明显（P〈0.01）,同时放射治疗后涂生理盐水组DMFT、DMFS明显高于氟化钠涂膜组（P〈0.01）。放射治疗后随访8~12个月,35例患者中死亡1例,失访2例。结论头颈部肿瘤患者在放射治疗前于牙釉质表面涂布氟化钠涂膜能显著降低放射性龋齿的发生。Objective To examine the effect of fluoride varnish on the tooth surface of the patients with head and neck cancer receiving radiotherapy and explore the preventative effect of fluoride varnish for radioactive caries.Methods 35 patients with head and neck cancer to receive radiotherapy were selected and divided into the fluoride varnish group and the sodium chloride group by self-control.NaF and NaCl were varnished 1,3,5 d before the radiotherapy.The decay missing filling tooth（DMFT）and decay missing filling surface（DMFS）in dental caries were examined before and 18 months after the radiotherapy.Results Before the radiotherapy,DMFT and DMFS of the two groups showed no significant difference（P0.05）.However,18 months after radiotherapy,DMFT and DMFS of two groups increased significantly（P0.01）and more increase took place in the sodium chloride group（P0.01）.After radiotherapy,MFT and MFS of the sodium chloride group were much greater than those of the fluoride varnish group（P0.01）.After radioactive therapy,patients were followed up for 8-12 months,1 case died and 2 cases lost follow-up among the 35 patients.Conclusion Coating of Fluoride varnish on enamel surface of the patients with head and neck cancer before radiotherapy can significantly reduce the occurrence of radioactive caries.
