美容切口在颧骨颧弓骨折内固定中的应用

摘　　要：目的：以美容的观点探讨颧骨颧弓骨折手术入路的切口设计。方法：对30例颧骨颧弓骨折的患者应用经头皮半冠状切口、上前庭沟和睑缘下切口,行微型钛板骨折坚固内固定术。结果：30例患者中1例出现轻度睑外翻,2周后消失;1例轻度眼睑水肿,3周后消失;1例出现眶下区麻木,术后1~2个月后恢复正常。所有病例均为一期愈合,6个月后复查均未能发现明显瘢痕。结论：睑缘下切口配合头皮冠状切口、口内前庭沟切口在颧骨颧弓骨折中的应用,切口隐蔽、美观、术野暴露良好,内固定操作方便快捷,术后美观效果好,在临床上应用广泛。Objective A modified incision was used in the patients with zygoma and zygomatic arch fracture to reduce the visible scar.Methods 30 patients with zygoma and zygomatic arch fracture were included in this study.A modified incision（semi-coronal scalp incision,vestibular sulcus incision,eyelid incision）was applied to the patients for aesthetic effects in the rigid internal fixation operation and preservation of facial nerve as well as infraorbital nerve.Observe the cosmetic effect of this incision and the clinical outcome.Results The modified incision healed well in all patients.All the patients were highly satisfied with this modified incision,and there were not visible scar after 6 months.One patient complained of ectropion,and one complained of eyelid edema,but recovered 2 weeks and 3 weeks postoperatively respectively.One patient complained of numbness in the infraorbital area,but recovered 1～2 months postoperatively.Conclusion The modified incision can be applied to the patients with zygoma and zygomatic arch fracture during operation and ideal postoperative aesthetic effect was achieved.
