电针酸灌注食管炎猫足三里穴对下食管括约肌调节功能的研究

摘　　要：目的研究电针足三里穴对外源酸制备的食管炎猫食管动力的影响,探讨足三里穴与食管运动调节之间的关系。方法将健康家猫32只均行内镜检查,确定无镜下食管病(炎)。随机分为健康组、食管炎组。应用食管滴注盐酸方法,建立食管炎模型,并依据食管炎症内镜诊断标准分级。应用加拿大E-7-1-1-1-2-5-5-5型8通道细微注水测压管和荷兰UPS-2020型食管测压系统测定猫下食管括约肌压力(LESP)。结果食管炎组内镜诊断标准分级:Ⅰ级2例,Ⅱ级8例,Ⅲ级6例。食管炎组电针前LESP为(8.25±5.75)mmHg(1 mmHg=0.133 kPa),电针期LESP升高为(10.88±6.42)mmHg(P=0.07),电针停止后LESP显著升高为(16.50±8.12)mmHg(P<0.05)。2组升高程度比较无显著性差异(P=0.12)。电针不同程度食管炎猫足三里穴对LESP影响不同:电针轻度和中度食管炎猫足三里穴LESP明显高于重度食管炎猫。结论电针健康猫和酸灌注食管炎猫足三里穴可升高LESP,电针对于不同程度的酸灌注食管炎症产生LESP影响存在差异。电针足三里穴对食管炎有治疗意义,可能对不同程度食管炎的疗效不同。足三里穴对食管运动影响的机制尚未完全阐明。Objective It is to study the influence of electroacupuncture （EA） at Zusanli point on esophageal dynamia of eats with esophagitis induced by eetogenous acid and to approach the relationship between Zusanli point and esophageal motility regulation. Methods After endoscope examination, 32 cats were confirmed as healthy ones without esophagitis. All experimen- tal cats were randomly divided into healthy group and esophagitis group. Esophagitis model was established with hydrochloric acid dripping. The esophagitis group was graded with esophagitis endoscope diagnostic code. The lower esophageal sphincter pressure （LESP） was measured and recorded with E - 7 - 1 - 1 - 1 - 2 - 5 - 5 - 5 type 8 Channel Fine Infusion Piezometric Tube （Canada） and UPS -2020 type Esophageal Pressure Measurement System （Netherlands）. Results In esophagitis group （2 ease of grade I, 8 cases of grade II and 6 cases of grade III esophagitis） , the LESP increased from （8.25 ±5.75） mmHg to （10.88±6.42） mmHg （P=0.07） during EA and reached （16.50±8.12） mmHg （P〈0.05） after EA. There was no significant difference between the two groups on the scale of EA LESP pressure rising （P = 0.12）. The degrees of pressure rising of mild and moderate esophagitis cases were both higher than that of severe esophagitis cases in esophagitis group varied. Conclusion EA on Zusanli point of both healthy and esophagitis eats can cause obvious LESP rising and lead to wholesome regulation on LESP. The degree of pressure rising is varied in different degree esophagitis and will lead to different curative effect. However, the mechanism remains unclear.
