生物羊膜在难治性青光眼复合式小梁切除术中的临床观察

摘　　要：目的观察生物羊膜应用于复合式小梁切除术治疗难治性青光眼的临床疗效。方法难治性青光眼患者44例52只眼,随机分为羊膜移植组和对照组,羊膜移植组术式在复合式小梁切除术(丝裂霉素+可调整缝线)基础上联合应用生物羊膜移植,对照组只采用复合式小梁切除术。所有患者术后均随访12个月,观察手术前后眼压变化、术后功能性滤过泡形成。结果羊膜组术后12个月眼压的控制、完全成功率及功能性滤过泡形成均明显优于与对照组,差异均具有显著性(P<0.05)。结论生物羊膜应用于复合式小梁切除术中有利于形成功能性滤过泡,有效地控制眼压。Objective To observe the curative effect of biology amniotic membrane on combined trabeculectomy in refractory glaucoma.Methods 52 eyes of 44 patients with refractory glaucoma were involved in this prospective trial,these patients were divided into two groups randomly,amniotic membrane group and control group.Amniotic membrane group combined with amniotic membrane on the basis of compound trabeculectomy（mitomycin and adjustable suture）,control group only combined trabeculectomy.All the patients were followed up for 12 months.The outcome＇s evaluation included： vision,intraocular pressure before and after surgery,the functional ltration blebs.Results The intraocular pressure,the functional ？ltration blebs and the completely success rate got better in amniotic membrane group than in control group with remarkably significance（P0.05） at 12 months after surgery respectively.Conclusion The combination of compound trabeculectomy and biology amniotic membrane is more favorable to formation of functional ltration blebs,and can reduce the intraocular pressure significantly.
