非酒精性脂肪肝患者脂代谢紊乱与血清补体C3的相关性研究

摘　　要：目的:研究血清补体C3与非酒精性脂肪肝发病危险因素及非酒精性脂肪肝发生发展的相关性。进而了解C3在非酒精性脂肪肝患者血清中含量变化的意义。方法:40例健康对照组、40例单纯转氨酶升高组、40例非酒精性脂肪肝转氨酶正常组、40例非酒精性脂肪肝转氨酶升高组。按脂肪肝患者ALT是否升高分为转氨酶正常组和异常组。用酶联免疫吸附试验测定入选者血清中补体C3的含量,以及生化方法检测甘油三酯(TG)、丙氨酸氨基转移酶(ALT)、γ-谷氨酰转移酶(GGT)、低密度脂蛋白(LDL)的水平,测量身高、体重、腹围、臀围,并计算体重指数(BM I)及腰臀比(WR)。结果:脂肪肝转氨酶正常组血清补体C3的水平与正常对照组比较有极显著差异(P<0.001);组间比较有显著差异(P<0.05)。脂肪肝患者血清TG、BM I与NC组比较均有显著差异(P<0.05)。结论:补体C3是非酒精性脂肪肝诊断的血清学指标之一,可以判断非酒精性脂肪肝病变程度,做为临床指标动态观察脂肪肝严重程度及判断其转归的指标,也为脂肪肝的治疗提供新的思路。Objective To investigate the role of serum C3,hyperlipemia in the lipid metabolism disorder of nonalcoholic fatty liver（NAFLD） and its clinical significance.Methods Serum C3 level was detected by ELISA in 40 patients with NAFLD with normal ALT,40 patients with NAFLD with abnormal ALT,40 patients only with abnormal ALT and 40 patients with normal control（NC）.Total cholesterol（TC）,triglyeride（TG）,low density lipoprotein（LDL） and γ-glutamyl transferase（γGT） levels were detected by biochemical methods,Height,weight,abdominal circumference and hip circumference were measured too.Results Serum C3 level was significantly higher in NAFLD with normal ALT（P0.001） and 40 patients with NAFLD with abnormal ALT（P0.001） than that in NC group,respectively.Moreover,there was also significant difference between two groups（P0.05）.These indicators of serum LDL,TG,and BMI in patients with fatty liver were significant higher than that in NC group（P0.05） respectively and a positive linear correlation existed between serum C3 and TG,BMI in patients with fatty liver.Conclusion Complement C3 play an important part in the lipid metabolism disorder of NAFLD.Serum C3 level in patients with NAFLD was closely related to the lipid metabolism disorder of liver,which could serve as an indicator of secondary diagnosis of NAFLD and determine the severity of liver lipid metabolism disorder.Targeted application of complement C3 antibody would be expected to be an effective treatment of intervention in patients with NAFLD.
