摘  要:目的探讨急性脑梗死患者脑血流中微栓子与颈动脉粥样硬化斑块的关系。方法 67例急性脑梗死患者依据颈动脉超声检测分为颈动脉无斑块组(27例)和斑块组(40例),后者再分为不稳定斑块亚组(19例)和稳定斑块亚组(21例);对所有患者应用经颅多普勒仪进行脑血流中微栓子监测;比较各组间微栓子阳性率。结果脑血流中微栓子阳性率颈动脉斑块组(30%)显著高于无斑块组(3.7%),不稳定斑块亚组(47%)显著高于稳定斑块亚组(14.3%)(均P<0.05)。结论颈动脉粥样硬化脑梗死患者脑血流中微栓子阳性率高;不稳定斑块更易脱落形成微栓子。Objective To study the relationship between microembolism in cerebral blood flow and carotid atherosclerotic plaque in patients with acute cerebral infarction (ACI). Methods Sixty-seven ACI patients were divided into carotid artery no-plaque group (27 cases) and carotid artery plaque group (40 cases), the latter was including unsteady plaque subgroup (19 cases ) and steady plaque subgroup (21 cases) , according to the results of carotid artery ultrasonic examination. The microembolism monitoring in cerebral blood flow of all the patients was undergone by transeranial doppler. The positive rate of mieroembolism in cerebral blood flow was compared among the groups. Results The positive rate of mieroembolic in cerebral blood flow in carotid artery plaque group (30%) was significantly higher than no-plaque group (3. 7% ), and which in the unsteady plaque subgroup (47%) was significantly higher than steady plaque subgroup ( 14.3% ) ( all P 〈 0. 05 ). Conclusions The positive rate of microembolism in cerebral blood flow in ACI patients with carotid atherosclerotic plaques is high. The unsteady plaque is much easier to generate microembolism.
