摘　　要：目的 探讨抗凋亡基因TOSO基因在慢性淋巴细胞白血病（CLL）CD19^＋B细胞中的表达情况及其临床意义.方法 实时定量PCR（绝对定量探针法）检测2006年3月至2010年9月中国医学科学院血液学研究所血液病医院收治的85例CLL患者CD19^＋B细胞亚群TOSO表达水平,分析CLL患者免疫球蛋白重链可变区（IGVH）突变状态、zeta链相关蛋白-70（ZAP70）和CD38的表达情况.结果 85例CLL患者CD19^＋B细胞TOSO表达水平明显高于15名健康对照（8.30±2.99比6.63±1.22,P=0.036）,亦显著高于15例其他B淋巴细胞增殖性疾病（7.12±1.13,P=0.023）.IGVH未突变组患者TOSO的表达水平显著高于突变组患者（9.87 4±1.08比7.61±3.03,P=0.000）.Binet分期A、B、C期患者TOSO的表达水平分别为（7.27 4±2.83,8.73 4±1.86,9.91 4±3.03）,差异有统计学意义（P=0.000）.需要治疗组TOSO表达水平显著高于观察组（9.37 4±2.12比7.19 4±3.23,P=0.001）.53例年龄≧55岁的患者TOSO的表达水平显著低于32例〈55岁患者（7.95±3.22比9.10 4±2.06,P=0.049）.ZAP70、C目的 探讨抗凋亡基因TOSO基因在慢性淋巴细胞白血病（CLL）CD19＋B细胞中的表达情况及其临床意义.方法 实时定量PCR（绝对定量探针法）检测2006年3月至2010年9月中国医学科学院血液学研究所血液病医院收治的85例CLL患者CD19＋B细胞亚群TOSO表达水平,分析CLL患者免疫球蛋白重链可变区（IGVH）突变状态、zeta链相关蛋白-70（ZAP70）和CD38的表达情况.结果 85例CLL患者CD19＋B细胞TOSO表达水平明显高于15名健康对照（8.30±2.99比6.63±1.22,P=0.036）,亦显著高于15例其他B淋巴细胞增殖性疾病（7.12±1.13,P=0.023）.IGVH未突变组患者TOSO的表达水平显著高于突变组患者（9.87 4±1.08比7.61±3.03,P=0.000）.Binet分期A、B、C期患者TOSO的表达水平分别为（7.27 4±2.83,8.73 4±1.86,9.91 4±3.03）,差异有统计学意义（P=0.000）.需要治疗组TOSO表达水平显著�Objective To investigate the expression and its prognostic significance of TOSO in CD19＋B cells from Chinese chronic lymphocytic leukemia（CLL）patients.Methods The expression of TOSO was detected by absolute quantitative reverse transcription-polymeric chain reaction（RT-PCR）in a cohort of 85 untreated CLL patients from March 2006 to September 2010.Their status of immunoglobulin heavy chain vailable（IGVH）somatic mutation,ZAP70 and CD38 were analyzed.Resuits The expression of TOSO was significantly elevated in CLL patients versus the healthy population（8.30 4±2.99 vs 6.63 4±1.22.P=0.036）and other B cell lymphoproliferative diseases（8.30±2.99 vs 7.12±1.13,P：0.023）.The expression of TOSO was elevated in the IGVH nou-mutated group versus the mutated group（9.87 4±1.08vs 7.61 4±3.03.P=0.000）.The expression of TOSO was significantly elevated in Binet stage C patients versus in Binet stage B and Binet stage A patients（9.91 4±3.03 vs 8.73±1.86 vs 7.27 ±-2.83,P=0.000）.The expression of TOSO rose to（9.37±2.12）in the chemotherapy group.And it was significantly higher than that in the observation group（7.19±3.23,P=0.001）.At the cut-off point of 55 years old,the younger patients had a higher expression of TOSO than the elders（9.10±2.06 vs 7.95 4±3.22,P=0.049）.The expression of TOSO in ZAP70 and CD38-positive groups had no difierence with those in the negative groups.Conclusion The over-expressed TOSO in CLL cells iS associated with a progressive disease.It may be an important prognostic factor for CLL.
