摘 要:目的探讨磁敏感加权成像(SWI)在急性期缺血性脑梗死中的应用。方法在1.5T超高场MR扫描仪上,运用常规MRI序列、DWI、MRA和SWI序列对15例急性期缺血性脑梗死患者的MR表现进行综合评价。SWI采用3DFLASH序列,对原始图像进行最小强度投影重建以显示脑内整体的静脉情况。结果15例急性期脑梗死病灶在DWI上均呈高信号,MRA上2例显示一侧大脑中动脉的闭塞,13例显示动脉血管不同程度狭窄。SWI上见9例梗死区静脉血管减少,1例静脉血管完全消失。15例中有3例显示脑梗死病灶引流静脉内部显著低信号,3例除梗死病灶外,另可见脑内的微出血灶,2例合并出血,1例显示动脉内血栓与MRA上所示动脉阻断位置大致吻合。结论SWI技术在急性期缺血性脑梗死的诊断和评估中具有很大的价值。Objective To evaluate the appli cation of MR susceptibility-weighted imaging (SWI) in acute ischemic cerebral infarction. Methods Fifteen patients with acute cerebral infarction were studied using a 1.5T MR scanner. All patients underwent MR imaging with the following sequence: conventional MRI, DWI, MRA and SWI. The SWI images were acquired by 3D-FLASH sequence and transferred to the workstation and reconstructed to display the cerebral veins by the minimum intensity projection. Results The loci of acute cerebral infarction were hyperintense on DWI in all cases, MRA showod obstruction of middle cerebral artery (MCA)in 2 cases and stenosis in varying degrees of MCA in 13 cases. SWI demonstrated reduction of cerebral vein distribution within infarction area in 9 cases and absence in 1 case, prominent hypointense in draining vein with mierobleeds in 3 cases, accompanied with hemorrhage in 2 cases,the thrombus at right MCA showed by SWI was consistent with the appearance on MRA. Conclusion SWI is useful for the diagnosis and evaluation of the acute cerebral infarction.
