摘　　要：目的探讨下肢滑膜肉瘤的临床和CT特点，提高对本病的诊断水平。方法对发生在下肢的6例原发和7例复发滑膜肉瘤的临床和CT资料进行回顾性分析。结果患者首次就诊时平均年龄38。6岁。13例患者共有15个病灶，4个位于大腿，4个位于足部（包括踝关节），4个位于腹股沟区、2个位于胭窝，1个发生于臀部。在CT图像上2个表现为多结节状，8个为圆形或椭圆形肿块，5个为分叶状或不规则形，4个破坏邻近骨质，5个接触邻近骨但未对其破坏。3个病灶边缘清楚，12个病灶部分或全部边缘不清。所有病灶平扫密度均稍低于邻近肌肉或对侧相同部位软组织密度，其中8个病灶密度均匀，5个密度不均，2个呈囊性。增强后8个病灶明显不均匀强化，4个不均匀轻度强化，1个轻度均匀强化，2个囊性病灶分隔及边缘强化。结论下肢滑膜肉瘤的CT表现具有一定特征。Objective To investigate the clinical and CT features of the synovial sarcoma of lower extremity, so that to improve the diagnostic knowledge of this disease. Methods The clinical and CT data of 6 cases with primary and 7 cases with recurrent synovial sarcoma of lower extremities were analyzed retrospectively. Results The average age of patients at first visiting time was 38.6 years. There were totally 15 lesions in 13 patients. The lesions located in the thigh in 4,the foot （including the ankle joint）in 4,the groin area in 4, the popliteal fossa in 2 and buttock in one. On CT images, the lesions appeared as multi-nodus in 2, round or ellipse in 8 and lobulated or irregular in 5. The bones adjacent to the lesions were destructed in 4. The borderlines of the lesions were definite in 3, partial or overall undefinite in 12. The density of all lesions was slightly lower than that of the neighboring muscle or the soft tissue of the same parts in the opposite side,and the density of the lesions was uniform in 8, uneven in 5,2 lesions showed cystic degeneration. At contrast-enhanced scan,the lesions displayed obvious heterogeneous enhancement in 8, slightly heterogeneous enhancement in 4 and mild homogeneous enhancement in one and cystic septa and the margins of the lesions enhanced in 2. Conclusion CT manifestations of synovial sarcoma of lower extremity are of certain characteristic.
