成人骨性下颌偏斜与脊柱冠状面形态及躯干平衡性的相关性研究

摘　　要：目的研究成人骨性下颌偏斜患者脊柱冠状面形态及躯干平衡性。方法对35例成人骨性下颌偏斜患者及10例正常对照者拍摄站立位脊柱全长正位片和头颅后前位片，并对相关数据进行测量分析。结果①下颌偏斜者脊柱冠状面形态均不同程度呈“S”型弯曲，正常对照组脊柱基本呈一直线与重力线平行，两者差异有统计学意义（P〈0．01）；②下颌偏斜患者的躯干平衡性均为失平衡（失衡角〉1°），正常对照者躯干平衡性均在正常范围内（失衡角〈1°），两者差异有统计学意义（P〈0．01）；③下颌偏斜患者的脊柱侧弯程度和肩失平衡程度与下颌偏斜程度均呈正相关，并呈线性趋势；④下颌偏斜方向与颈椎段侧弯方向具有异向性，与躯干段侧弯方向具有同向性。结论骨性下颌偏斜与脊柱形态和躯干平衡性之间有一定的相关性，临床中应引起足够重视。Objective To study the morphology of spine coronal plane and trunk balance in adult patients with mandibular deviation and find the difference between them and normal samples. Methods 35 cases of adult patients with skeletal mandibular deviation and 10 cases of normal occlusion were taken full spine films and skull anteroposterior films. The data were measured and analyzed. Results @ The spin coronal morphology of mandibular deviation demonstrated ＂S＂ type curve, the control group showed the straight line parallel with the gravity line, both have the statistically significant difference （P〈0.01）; （1） The balance of mandibular deviation were disturbed （imbalance angle〉 1°）, the balance of normal occlusion were within the normal range （imbalance angle 〈1°）, there was significant difference between them （P〈0. 01 ） ; （2） The degree of scoliosis and shoulder imbalance were correlated with the degree of mandibular deviation, and presented a linear trend ; （4） Cervical segment bending of spin was different with the direction of mandibular deviation but the trunk bending section of spin has the same orientation with it. Conclusions The mandibular deviation has closely correlated with various degrees of scoliosis and trunk imbalance. Great attention should be paid and active treatment is necessary.
