摘 要:目的探讨血清脑源性神经营养因子(BDNF)水平在双相Ⅰ型(BDⅠ型)、双相Ⅱ型(BDⅡ型)患者与正常对照者间的差异。方法对广州市3家三级甲等医院精神科临床诊断为情感性精神障碍的患者使用SCID-I/P再诊断,符合BDⅠ型和BDⅡ型诊断标准的患者为研究组,并进行YMRS及MADRS评定。对照组为经SCID-I/P排除符合DSM-IV轴I障碍的人员。用ELISA方法测定所有研究对象的血清BDNF水平。结果患者血清BDNF水平显著低于正常对照[(17.60±11.24)ng/ml vs(26.04±12.85)ng/ml,t=-3.953,P<0.001)];未发现BDⅠ型与BDⅡ型患者间血清BDNF水平存在统计学差异;未发现患者血清BDNF水平与YMRS、MADRS评分间存在相关关系;合用与未合用情感稳定剂患者间血清BDNF差异无统计学意义。结论 BDNF可能作为BD的生物学标记,在BDⅠ型与BDⅡ型障碍的病理生理学上起重要作用。Objective To explore the differences of serum BDNF level among bipolar disorder type Ⅰ ( BD Ⅰ), bipolar disorder type Ⅱ ( BD Ⅱ) and healthy controls. Methods A total of 156 patients with clinical diagnosis as affective disorder recruited from three hospitals were re - diagnosed with SCID - I/P. 72 patients with BD Ⅰ and 39 patients with BD Ⅱ composed the study sample and then were assessed with YMRS and MADRS. All the axis Ⅰ disorders in DSM - Ⅳ were excluded from the healthy controls by means of SCID - I/P. The serum level of BDNF was measured with the method of ELISA. Results Serum level of BDNF in all the 156 patients was significantly lower than that in healthy controls [ ( 17.60 ±11.24) ng/ml vs (26.04 ±12.85 ) ng/ml, t = - 3. 953, P 〈 0. 001 ]. The serum BDNF level showed no significant difference between patients with BD I and patients with BD Ⅱ. The correlations between serum BDNF level with scores of YMRS, MADRS were negative. There was no significant difference in serum BDNF level between patients with and without the treatment of mood stabilizers. Conclusion BDNF may be a biomarker of BD and play a important role in the pathophysiology of BD Ⅰ and BD Ⅱ.
