摘  要:目的探讨基质细胞衍生因子-1α(SDF-1α)、骨桥蛋白(OPN)、变异的CD44受体(CD44v6)和抗酒石酸酸性磷酸酶5b(TRACP-5b)在多发性骨髓瘤(MM)发病中的作用。方法用ELISA法检测了32例MM患者和15例对照骨髓单个核细胞(MNCs)和骨髓基质细胞(BMSCs)的SDF-1α、OPN、CD44v6和TRACP-5b水平。结果 MNCs产生的SDF-1α、OPN、CD44v6水平在初发和复发组、病情稳定MM组和对照组显著下降(P<0.05)。9例初发和复发患者的BMSCs与人骨髓瘤细胞系U266加入rhIL-6混合培养后,SDF-1α水平明显增高(P<0.05)。SDF-1α、OPN、TRACP-5b水平在MM患者骨质破坏≥3处、<3处和对照组显著下降(P<0.05)。结论 SDF-1α、OPN和CD44v6水平升高与MM发病或病情进展、骨质破坏有关。Objective To detect the level of stromal derived factor 1α(SDF-1α),osteopontin(OPN),CD44 variant isoforms(CD44v6) and tartrate-resistant acid phosphatase 5b(TRACP-5b) in multiple myeloma(MM) and to explore their roles in genesis of MM.Methods Bone marrow mononuclear cells(MNCs) and bone marrow stromal cells(BMSCs) from 32 MM patients and 15 control subjects were investigated.The levels of SDF-1α,OPN,CD44v6 in the conditioned media and TRACP-5b in serum from MM patients were analyzed by ELISA.Results The levels of SDF-1α,OPN and CD44v6 in MNCs of untreated and relapsed patients,stable patients and control subjects were significantly decreased(P0.05).The low level of SDF-1α was also detected in BMSCs.When rhIL-6 was added to human MM cell lines U266 and BMSCs of 9 untreated and relapsed MM patients and cultured,there was significant increase in SDF-1α(P0.05).The levels of SDF-1α,OPN and TRACP-5b in ≥3 sites,and less than 3 sites of bone lesions and control subjects were significantly decreased(P0.05).Conclusions Increase in SDF-1α,OPN and CD44v6 activity is associated with progress or pathogenesis and bone lesions of MM.
