摘  要:目的研究角膜原位癌的组织病理学特性,探讨角膜上皮干细胞在角膜原位癌发生过程中的作用。方法收集8例角膜原位癌标本,常规石蜡包埋切片,HE染色,做形态学检查;并对其做P63和AE5免疫组化研究。结果病理检查发现角膜原位癌的癌细胞在上皮内高度增生并呈现明显的异形性,增生的细胞主要为基底细胞,上皮下基底膜完整。当用角膜上皮干细胞特异性标记物P63免疫组织化学染色,发现肿瘤区已发生癌变的恶性细胞,细胞核均染成棕色,但外围或表浅的已分化的梭形细胞未见表达,与其邻近的正常角膜上皮细胞也呈阴性反应。结论角膜原位癌细胞主要由表达P63阳性的肿瘤干细胞和少许不表达P63但表达角质蛋白的分化细胞构成,角膜原位癌起源于角膜上皮干细胞。Objective To investigate the histopathological features of corneal carcinoma in situ and significance of corneal epithelial stem cells in corneal carcinoma in situ.MethodsA total of 8 cases with corneal carcinoma in situ were collected,and these specimens were studied with normal paraffin section,HE staining and morphological examination.Immunohistochemical studies of P63 and AE5 were taken.ResultsThe normal pathologic examination had found that the tumor cells were with high proliferation in the epithelium and with obvious heterology;The proliferative cells were mainly corneal basal cells,and subepithelial basement membranes were complete.With immunohistochemical staining of P63,the specific marker in corneal epithelial stem cells,malignant cells were found in tumor area,the nucleus were stained with brown,and no expression was found in differentiated fusiform cells at surrounding or surface layer,and negative expression was found in neighbouring normal corneal epithelial cells.ConclusionsCorneal carcinoma in situ cells mainly include tumor stem cells with positive expression of P63 and differentiated cells with expression of keratoprotein,but with little expression of P63.Corneal carcinoma in situ is derived from corneal epithelial stem cells.
