摘　　要：目的评价玻璃体腔注入全氟丙烷(C3F8)气体治疗黄斑部视网膜下积血的疗效。方法选取黄斑部视网膜下积血患者23例(24眼)均行玻璃体腔注射C3F8气体0.3～0.4mL,注气后保持面向下体位1周以上,观察视力变化及眼底等情况。结果术后1周21眼黄斑区积血明显吸收或移位至黄斑区以外,视力有不同程度的提高,矫正视力由治疗前的眼前手动～0.06提高至0.04～1.00,其中2例(2眼)外伤性黄斑出血视力恢复最好,达1.0和0.8;1眼黄斑区积血明显吸收但视力无提高;1眼形成轻度玻璃体混浊,检眼镜下隐约见黄斑区积血已经完全吸收,视力由0.04提高至0.06;1眼黄斑区积血加重,形成出血性视网膜脱离及玻璃体混浊。术后2周时23眼黄斑区积血明显吸收或移位至黄斑区以外,矫正视力提高至0.06～1.00。结论玻璃体腔注气疗法是治疗黄斑部视网膜下积血的一种简单而有效的方法 ,适宜于黄斑部视网膜下积血面积较小者,尤其对外伤所致黄斑部视网膜下积血效果显著。Objective To evaluate the efficacy of intravitreal injection of perfluoropropane gas for macular subretinal hemorrhage.MethodsTwenty-four eyes of 23 cases suffering from macular subretinal hemorrhage were analyzed.All patients were treated with intravitreal injection of C3F8 gas with 0.3 mL to 0.4 mL,and maintained prone position at least for one week.The patients＇ visual acuity and ocular fundus were observed.ResultsOne week after the operation,the macular subretinal hemorrhage absorbed or position displaced away from macula in 21 eyes;Postoperative visual acuity improved in 21 eyes,and the corrected visual acuity had get 0.04 to 1.00 from hand movement to 0.06 before operation.Among them,two traumatic eyes＇ visual acuity achieved 1.00 and 0.80,respectively.The macular subretinal hemorrhage absorbed obviously but visual acuity without improvement in 1 eye.There was 1 eye with vitreous opacity,but its macular subretinal hemorrhage had been absorbed and with visual acuity from 0.04 to 0.06.Macular subretinal hemorrhage increased and retinal detachment had found in 1 eye.Two weeks after the operation,the macular subretinal hemorrhage absorbed or their position displaced away from macula in 23 eyes,and their corrected vision were from 0.06 to 1.00.ConclusionsIntravitreal injection of C3F8 gas is an effective and easy method for macular subretinal hemorrhage,it can be used for macular subretinal hemorrhage with small area,especially for traumatic macular subretinal hemorrhage.
