摘  要:目的验证小波分析在原发型开角性青光眼闪光视觉诱发电位特征提取中的可靠性。方法正常志愿者10人(10眼),确诊为原发性开角型青光眼患者12例(12眼),分别为正常组和青光眼组,2组均进行闪光视觉诱发电位测量。利用离散Meyer小波对原始信号进行7层小波分解,提取单一尺度小波系数,比较与原信号时域分析结果的一致性。对小波变换后的信号进行时域统计学分析。分析小波变换后信号的P1、N2、P2峰潜时,N1P1、P1N1波幅值。结果闪光视觉诱发电位主要成分为N1、P1、N2、P2、N3五波,其中原始信号指标正常组N1P1为(8.1±3.0)μV,青光眼组N1P1为(6.5±8.2)μV,2组差异无统计学意义(P=0.31);原始信号指标正常组P1N2为(12.5±9.1)μV,青光眼组P1N2为(12.2±4.3)μV,2组差异也无统计学意义(P=0.21)。原发性开角型青光眼患者可提炼出第4、5层细节系数和(D4+D5),其波形变化较正常眼显著,波形规整,主要波形成分识别度高,重构小波细节系数D45指标后正常组N1P1为(9.2±4.2)μV,青光眼组N1P1为(6.5±3.8)μV;正常组P1N2为(13.7±5.6)μV,青光眼组P1N2为(10.2±3.2)μV,N1P1和P1N2幅值差异在正常眼和原发性开角型青光眼间均有显著统计学意义(均为P<0.01)。结论小波分析可作为提取正常和原发性开角型青光眼闪光视觉诱发电位特征的可靠方法 。Objective To evaluate the reliability of wavelet analysis on the feature extraction of flash visual evoked potential in primary angle-open glaucoma.MethodsWe selected 10 normal controls(10 eyes)as normal group and 12 cases with primary open angle glaucoma(12 eyes)as glaucoma group.Flash visual evoked potential was measured for patients in both two groups.The original data was decomposed into 7 levels by discrete Meyer wavelet,and the wavelet coefficient of each level was extracted to compare the consistency with time domain analysis results of the original signal.The latency of P1,N2,P2 and the amplitude of N1P1,P1N1 after wavelet transform were measured.ResultsThe main components of the flash visual evoked potential were N1,P1,N2,P2 and N3.The original data of N1P1 were(8.1±3.0)μV in normal group and(6.5±8.2)μV in glaucoma group.No statistical significant was found between the two groups(P=0.31).The original data of P1N2 were(12.5±9.1)μV in normal group,and(12.2±4.3)μV in glaucoma group.No statistical significant was found between the two groups(P=0.21).The sum of the forth and the fifth detail coefficients and(D4+D5)could be extracted in patients with primary angle-open glaucoma.Its waveform was regular and easily distinguished,and with more remarkable change compared with control eyes.The N1P1 were(9.2±4.2)μV in normal group and(6.5±3.8)μV in glaucoma group after the reconstruction of wavelet detail coefficients D45.There were statistically significant difference in the amplitude of N1P1 and P1N2 between normal eyes and eyes with primary angle-open glaucoma(both P0.05).ConclusionWavelet analysis can be used as a reliable method to extract the flash visual evoked potential of normal eyes and eyes with primary angle-open glaucoma.
