乳腺癌原发灶与腋淋巴结转移灶ER和PR及c-erbB-2表达的对照研究

摘　　要：目的:研究雌激素受体(ER)、孕激素受体(PR)和c-erbB-2在乳腺癌原发灶和同期腋淋巴结转移灶之间的表达差异。方法:免疫组织化学方法检测60例初治单侧乳腺癌同期腋窝淋巴结转移手术治疗患者ER、PR和c-erbB-2在原发灶及腋淋巴结转移灶的表达差异。结果:ER在乳腺癌原发灶中的阳性率为56.7%(34/60),腋淋巴结转移灶中为48.3%(29/60);PR在乳腺癌原发灶中的阳性率为55.0%(33/60),腋淋巴结转移灶为48.3%(29/60);c-erbB-2在乳腺癌原发灶中的阳性率为26.7%(16/60),腋淋巴结转移灶为28.3%(17/60);ER、PR和c-erbB-2的阳性率在两者之间的差异均无统计学意义。ER在原发灶和腋窝淋巴结转移灶之间总的变化率为21.7%(13/60),PR为16.7%(10/60),c-erbB-2为15.0%(9/60)。结论:ER、PR和c-erbB-2的表达在乳腺癌原发灶和腋窝淋巴结转移灶之间存在不一致现象,但无统计学意义。判断乳腺癌的预后要综合考虑其原发灶和转移灶的生物学特性。OBJECTIVE： To investigate the discordance of expressions of ER, PR and c-erbB-2 statuses between primary and simultaneous metastatic axillary lymph nodes of breast cancer. METHODS： Expressions of ER, PR and c-erbB-2 were detected in primary and simultaneous metastatic axillary lymph nodes in 60 cases of breast cancer with immunohistochemisty. RESULTS： Positive rates in primary focuses and simultaneous metastatic axillary lymph nodes were 56.7 （34/60） and 48.3% （29/60） for ER, 55.0%（33/60） and 48.3%（29/60） for PR,26.7%（16/60） and 28. 3% （17/60） for c-erbB-2. All the posititive rates of them had no significant difference between primary and simultaneous metastatic axillary lymph nodes. Overall the percentage of discordance mark status in the primary tumor and its metastatic lymph nodes was 21. 7% （13/60） for ER, 16.7%（10/60） for PR and 15.0%（9/60） for c-erbB-2. CONCLUSIONS： Expressions of ER, PR and c erbB-2 were discordant in some cases,but all of the discordances were not significant. Biological characteristics in primary and metastasis of breast cancer should be synthetically considered to predict the prognosis of patients.
