幽门螺杆菌感染与胃粘膜肠化生的关系

摘　　要：为探讨Hp 感染与胃粘膜肠化生及其增殖状态分型、硫酸化粘液分泌状况的关系,采用甲苯胺蓝染色法检测2 800 例胃粘膜活检组织及外科手术标本中Hp 的感染,并对187 例伴肠化生慢性萎缩性胃炎作免疫组化与粘液组化套染,进行肠化生增殖状态分型。结果显示胃粘膜出现肠化生和萎缩、癌变时,Hp 的检出率显著降低;Hp 感染与肠化生细胞是否分泌硫酸粘液无关;高增殖型肠化生中Hp 的检出率显著高于中、低增殖型。提示Hp 感染不仅在肠化生的发生中起作用,而且对肠化生细胞的增殖起促进作用,促进肠化生向异型增生过渡。To find out the relationship among Helicobacter pylori(Hp) infection,proliferation types of gastric mucosa and the expression of vitriolic mucus,toluidine blue staining was used to detect Hp in 2 800 cases of gastric biosy specimens or surgical specimens.By immunohistochemical and mucus histochemical methods,187 cases of atrophic gastritis with intestical metaplasia(IM) were stained to identify the types of gastric mucosa proliferation. Results showed that when IM,atrophy and cancer occured in gastric mucosa,the prevalence of Hp was significantly decreased;that Hp infection was not related to the expression of vitriolic mucus in IM cell;and that the pvevalence of Hp in high proliferation type(HPT) was significantly higher than that in middle or low proliferation type(MPT or LPT).It seems that Hp infection not only plays a part in the occur of IM,but also contributes to the development of the abnormal proliferation types.
