摘　　要：目的分析Krukenberg瘤患者的临床病理特点及治疗,寻找预后相关因素。方法回顾性分析34例确诊为Krukenberg瘤且有完整生存资料者的临床病理特征,对生存相关参数进行单因素及多因素分析。结果年龄14～73岁,中位年龄41岁;原发瘤包括胃癌20例(58.8%),结肠癌6例(17.7%),直肠癌7例(20.6%),不明者1例(2.9%);单侧卵巢转移12例(35.3%);双侧卵巢转移22例(64.7%);全组患者生存时间为0～55个月,中位生存11.0个月。单因素分析显示,年龄≤50岁者预后较差(P=0.011),化疗≥4周期者生存期较长(P=0.006);多因素分析显示仅年龄与预后相关(P=0.005)。结论 Krukenberg瘤患者预后差,目前尚未发现有效化疗方案,绝经后女性、经积极手术治疗并完成≥4周期化疗者预后相对较好。Objective To analyze the clinicopathological features and treatment of patients with Krukenberg tumor,so as to elucidate key factors related to treatment efficacy and survival.Methods The clinicopathological features of 34 cases of Krukenberg tumor with complete follow-up information were retrospectively studied,with emphasis on therapeutic modalities and prognosis.Multivariate analyses on survival related parameters were conducted.Results The age distribution of 34 cases was 14-73 years（median 41）.The primary tumors were gastric cancer in 58.8%（20/34）,colon cancer in 17.7%（20/34）,rectal cancer in 20.6%（7/34）,and unknown primary origin in 2.9%（1/34） of the cases.Ovarian lesions were bilateral in 22 cases（64.7%） and unilateral in 12 cases（35.3%）.Survival was 0-55 months（median 11.0）.Survival was closely related to age and cycles of chemotherapy on univariate analysis（P=0.011 and P=0.006,respectively）.Survival of those patients who were 50 years and got ≥4 cycles of chemotherapy was better.Only age was related to survival on multivariate analysis（P=0.005）.Conclusion There is no evidence of any good approaches to Krukenberg tumor,but post-menopausal women who have completed at least 4 cycles of chemotherapy may have improved outcome.
